Diagnostic value of two-dimensional ultrasound combined with contrast-enhanced ultrasound in thyroid TI-RADS 3-5 nodules

摘  要:目的探讨二维超声联合超声造影检查对甲状腺影像报告与数据系统(TI-RADS)3~5类结节的诊断价值。方法选取甲状腺结节患者300例共419个结节(TI-RADS 3~5类)为研究对象,采用飞利浦IU-22型彩色多普勒超声诊断仪行二维超声检查和超声造影检查,均进行病理检查。以病理检查为金标准,对比二维超声、超声造影及二维超声联合超声造影诊断TI-RADS 3~5类结节的阳性预测值、阴性预测值及诊断敏感度、特异度、准确度。结果二维超声显示TI-RADS 3、4、5类结节分别为138个、174个、107个。病例检查证实良、恶性结节分别为284个、135个。超声造影提示恶性结节135个。超声造影的敏感度和阴性预测值较二维超声明显增加(P<0.05),而特异度、准确度、阳性预测值无显著性差异(P>0.05);二维超声联合超声造影的阳性预测值、阴性预测值、诊断敏感度、特异度、准确度均大于二维超声、超声造影(P<0.05)。结论二维超声联合超声造影检查对鉴别诊断甲状腺TI-RADS 3~5类结节良恶性有一定的临床价值,值得临床推广应用。Objective To evaluate the diagnostic value of two-dimensional ultrasound combined with contrast-enhanced ultrasound in thyroid imaging and reporting system(TI-RADS)3-5 nodules.Methods A total of 419 thyroid nodules(TI-RADS 3-5)were selected from 300 patients.Two-dimensional ultrasound and contrast-enhanced ultrasound were performed with Philips IU-22 color Doppler ultrasound diagnostic instrument,and the pathological results were examined.With pathological examination as the gold standard,the positive predictive value,negative predictive value,diagnostic sensitivity,specificity and accuracy of two-dimensional ultrasound,contrast-enhanced ultrasound and two-dimensional ultrasound combined with contrast-enhanced ultrasound in the diagnosis of TI-RADS 3-5 nodules were compared.Results Two-dimensional ultrasound showed 138,174 and 107 TI-RADS 3,4 and 5 types of nodules,respectively.The benign and malignant nodules were respectively 284 and 135.Contrast-enhanced ultrasound revealed 135 malignant nodules.The sensitivity and negative predictive value of contrast-enhanced ultrasound were significantly higher than those of two-dimensional ultrasound(P<0.05),but there was no significant difference in specificity,accuracy and positive predictive value(P>0.05);the positive predictive value,negative predictive value,diagnostic sensitivity,specificity and accuracy of two-dimensional ultrasound combined with contrast-enhanced ultrasound were higher than those of two-dimensional ultrasound and contrast-enhanced ultrasound(P<0.05).Conclusion Two-dimensional ultrasound combined with contrast-enhanced ultrasound has certain clinical value in differential diagnosis of benign and malignant thyroid TI-RADS 3-5 nodules,which is worthy of clinical application.
