无创正压通气下气管镜检查在低氧血症患者中的有效性及安全性研究

摘　　要：目的探讨低氧血症患者在无创正压通气下行气管镜检查的安全性及有效性。方法收集2016年1~11月于我院住院需接受气管镜检查、治疗并伴有低氧血症的患者30例,随机分为两组,每组15例。实验组在无创正压通气下行气管镜检查;对照组在鼻导管吸氧下行气管镜检查。分别观察两组气管镜检查前、操作过程中生命体征的指标及变化,观察两组患者一次性进镜率、终止率、并发症发生率等指标。结果两组患者进镜前后血氧饱和度、心率均发生变化,且实验组一次性进镜率更高(P均<0.05)。两组气管镜操作终止率比较,差异无统计学意义(P>0.05);两组患者均无相关并发症发生,且无死亡病例。结论低氧血症患者在无创正压通气支持下行气管镜更加安全有效。Objective To investigate the safety and efficacy of bronchoscopy in patients with hypoxemia under noninvasive positive pressure ventilation. Methods A total of 30 patients with hypoxemia who underwent bronchoscopy examination and treatment in our hospital from January to November 2016 were enrolled. They were randomly divided into two groups, with 15 patients in each group. The experimental group underwent bronchoscopy under noninvasive positive pressure ventilation. The control group underwent bronchoscopy under nasal catheter oxygen. The indexes and changes of vital signs before and during entering the bronchoscopy were observed respectively. The indexes of one-time access rate, termination rate and complication rate of the two groups were observed. Results The oxygen saturation and heart rate of the two groups were changed before and after entering the bronchoscopy, and the one-time access rate of the experimental group was higher(P all <0.05). There was no statistically significant difference between the two groups in the termination rate of tracheoscopy(P>0.05). And no retated complications occurred in the two groups, no death was found.Conclusion Bronchoscopy with noninvasive positive pressure ventilation is more safe and effective in the treatment of patients with hypoxemia.
