摘 要:目的探讨促性腺激素释放激素拮抗剂(GnRH-ant)方案对黄体中期子宫内膜免疫细胞数量的影响。方法分别收集自然周期、GnRH-ant与GnRH激动剂(GnRH-a)促排卵方案的黄体中期的子宫内膜,通过HE染色分析组织分期,运用免疫组化及数字化分析系统定量检测子宫内膜各免疫细胞的阳性细胞率。结果与自然周期相比,GnRH-ant与GnRH-a方案对患者子宫内膜分期无显著性影响(P>0.05)。GnRH-ant组和GnRH-a组子宫内膜CD56+NK细胞阳性细胞率[分别为(18.92±4.31)%和(25.21±6.87)%]显著高于自然周期组[(7.48±3.93)%](P<0.01),而GnRH-ant组和GnRH-a组间比较无显著性差异(P>0.05)。GnRH-ant组子宫内膜未成熟树突状细胞CD1a+和CD8+T细胞的阳性细胞率显著高于自然周期组(P均<0.05)。结论GnRH-ant方案对子宫内膜组织分期无显著影响,但可显著上调子宫内膜各免疫细胞的阳性细胞率,提示其可能影响子宫内膜容受性。Objective:To analyze the effect of GnRH antagonist(GnRH-ant)on the number of endometrial immune cells in the mid-luteal phase.Methods:Endometrial tissue of patients with the natural cycle,GnRH-ant and GnRH agonist(GnRH-a)were collected respectively.Endometrial morphology was analyzed by HE staining according to the Noyes standard,and the percentages of immune cells in the endometrium were quantitatively analyzed by immunohistochemistry and Vectra automatic pathological imaging quantitative analysis system.Results:There was no significant difference in endometrial morphology in GnRH-ant and GnRH-a group compared with natural cycle group(P>0.05).However,the percentages of CD56+NK cells in GnRH-ant and GnRH-a group[(18.92±4.31)%and(25.21±6.87)%,respectively]were significantly higher than that in natural cycle group[(7.48±3.93)%](P<0.01),while there was no statistical difference between the GnRH-ant and GnRH-a group(P>0.05).The percentages of immature dendritic cells CD1a+and CD8+T cells in the endometrium of GnRH-ant group were significantly than those in natural cycle group(P<0.05).Conclusions:GnRH-ant has no significant effect on endometrial morphology.It significantly elevated the percentages of immune cells in the endometrium,suggesting that GnRH-ant may affect endometrial receptivity.
