摘  要:目的探讨支气管哮喘患儿痰液中CXCR2、CCR1、CCL3、CCL2表达水平及与肺功能及气道炎症的关系。方法选取2017年1月至2018年4月接受治疗的支气管哮喘患儿123例,其中急性发作期患儿68例,缓解期55例,另选同期来院体检的健康儿童45例为对照组。对受试者进行肺功能检查,RT-PCR检测痰上清液中CXCR2、CCR1 mRNA表达水平,采用ELISA检测痰液中CCL2、CCL3、IL-4、IL-8、IFN-γ水平,分析支气管哮喘患儿痰液中CXCR2、CCR1、CCL2、CCL3与肺功能及与炎症相关因子IL-4、IL-8、IFN-γ的相关性。结果支气管哮喘患儿肺功能指标FEV1%及FEV1/FVC均显著降低,哮喘急性期FEV1%、FEV1/FVC显著低于哮喘缓解期和对照组,哮喘缓解期FEV1%、FEV1/FVC显著低于对照组,差异均有统计学意义(P<0.05);哮喘急性期痰液中CXCR2、CCR1、CCL2、CCL3、IL-4、IL-8显著高于哮喘缓解期和对照组,哮喘缓解期痰液中CXCR2、CCR1、CCL2、CCL3、IL-4、IL-8显著高于对照组(P<0.05);哮喘急性期痰液中IFN-γ显著低于哮喘缓解期和对照组,哮喘缓解期痰液中IFN-γ显著低于对照组(P<0.05);相关性分析显示,支气管哮喘患儿痰液中CXCR2、CCR1、CCL2、CCL3与肺功能FEV1%、FEV1/FVC均呈显著负相关;支气管哮喘患儿痰液中CXCR2、CCR1、CCL2、CCL3与促炎因子IL-4、IL-8呈显著正相关,CXCR2、CCR1、CCL2、CCL3与抗炎因子IFN-γ呈显著负相关;IL-4、IL-8与抗炎因子IFN-γ均呈负相关。结论支气管哮喘患儿痰液中CXCR2、CCR1、CCL2、CCL3高表达与患儿肺功能和气道炎症相关,其可为疾病的诊治提供一定的理论依据。Objective To explore the expressions of CXCR2, CCR1, CCL3 and CCL2 in sputum of children with bronchial asthma and their relationships with pulmonary function and airway inflammation. Methods Sixty eight children during acute attack of bronchial asthma and fifty five children at remission stage, who were treated from January 2017 to April 2018, were enrolled in research group. And forty five healthy children, who took physical examination in our hospital during the same period, were enrolled in control group. Lung function tests were performed on the subjects. The expression levels of CXCR2 and CCR1 mRNAs in sputum supernatant were detected by RT PCR. And the levels of CCL2,CCL3, IL-4, IL-8 and IFN-γ in sputum were detected by ELISA. The correlations between the levels of CXCR2, CCR1,CCL2, CCL3 in sputum and lung function, inflammation-related factors IL-4, IL-8 ,IFN-γ in children with bronchial asthma were analyzed. Results The levels of lung function indexes including FEV1% and FEV1/FVC in children with bronchial asthma were significantly decreased. The FEV11% and FEV1/FVC in acute stage were significantly lower than those in remission stage and those in control group, and the FEV11% and FEV1/FVC at remission stage were significantly lower than those in control group ( P <0.05). Moreover, the levels of CXCR2, CCR1, CCL2,CCL3, IL-4 and IL-8 in sputum of the patients at acute stage were significantly higher than those at remission stage and those in control group ( P <0.05). The levels of CXCR2, CCR1, CCL2,CCL3, IL-4 and IL-8 at remission stage were significantly higher than those in control group ( P <0.05). And the levels of IFN-γat acute stage were significantly lower than those in remission stage and those in control group, the levels of IFN-γ at remission stage were significantly lower than those in control group ( P <0.05). Furthermore, correlation analysis showed that the levels of CXCR2, CCR1, CCL2, CCL3 in sputum of children with bronchial asthma were negatively correlated with lung function FEV1% an
