摘 要:目的掌握目前常州市职业工作人群的血脂水平及血脂异常发生率,并分析其影响因素。方法选取2018年1月至2018年12月常州市疾病预防控制中心门诊部进行职业健康体检的5670例体检者为研究对象并检测其血脂水平,采用多因素logistic回归研究性别、年龄、接触工龄、血压、心率以及职业有害因素对血脂异常的影响。结果常州市5670名职业作业体检者总胆固醇(TC)水平为(4.65±0.87)mmol/L、甘油三酯(TG)水平为(1.03±0.78)mmol/L、高密度脂蛋白(HDL-C)水平为(1.21±0.20)mmol/L、低密度脂蛋白(LDL-C)水平为(2.47±0.64)mmol/L,总血脂异常率为28.6%。多因素logistic回归结果显示,男性(OR=1.484,95%CI:1.275~1.728,P<0.001)、年龄(OR=1.135,95%CI:1.041~1.238,P=0.004)以及血压异常(OR=1.318,95%CI:1.144~1.519,P<0.001)都与血脂异常有关;在职业有害因素岗位类别方面,接触噪声作业人群(OR=1.433,95%CI:1.202~1.707,P<0.001)的血脂异常率要高于管理岗位,其他有害因素作业岗位人群与管理岗位人群相比,血脂异常率的差异均无统计学意义(P>0.05)。结论对50岁以上男性、血压异常者以及接触噪声作业等血脂异常高发职业作业人群,应加强对其血脂的定期监测,控制降低其血脂水平,从而预防和减少心血管疾病发生,延长其职业工作寿命。Objective To investigate the incidence rate of dyslipidemia and the level of blood lipid in occupational health physical examination population in Changzhou City,and to analyze the influencing factors.Methods The blood lipid levels of 5670 physical examiners who conducted occupational health examination in the Changzhou Center for Disease Control and Prevention from January to December in 2018 were determined,multivariate logistic regression was used to analyze the data.Results The average level of TC,TG,HDL-C and LDL-C in 5670 physical examiners were(4.65±0.87)mmol/L,(1.03±0.78)mmol/L,(1.21±0.20)mmol/L and(2.47±0.64)mmol/L,respectively.The results of multivariate logistic regression showed that males(OR=1.484,95%CI:1.275~1.728,P<0.001),age(OR=1.135,95%CI:1.041~1.238,P=0.004)and abnormal blood pressure(OR=1.318,95%CI:1.144~1.519,P<0.001)were the risk factors of dyslipidemia.In terms of occupational harmful factors,the rate of dyslipidemia in people exposed to noise(OR=1.433,95%CI:1.202~1.707,P<0.001)was higher than that in management posts.There was no significant difference in the abnormal rate of blood lipid between the workers exposed to other harmful factors and those in the management posts(P>0.05).Conclusion Regarding to the high-risk population of abnormal blood pressure,noise work and male older than 50,we should strengthen the regular monitoring of blood lipids and control their blood lipid levels to prevent and reduce cardiovascular and cerebrovascular diseases,so as to extend their professional working life.
