临床护士职业认同的横断历史元分析研究

摘　　要：目的探讨2010年来我国临床护士职业认同情况随年代的变化趋势。方法针对2011-2018年期间发表,采用刘玲护士职业认同量表研究临床护士职业认同情况50篇文献进行横断历史元分析。结果临床护士总体职业认同总均分及认知评价、社交技能、自我反思维度得分均值与年代呈正相关(P<0.05),相关系数分别为0.13、0.22、0.14、0.14,职业认同总均分升高了0.41分,认知评价、社交技能、自我反思3个维度分别升高了17%、16%、12%;而社会支持、挫折应对维度与年代不相关(P>0.05)。亚群体总均分及其男、女护士、新护士社会支持维度和ICU护士认知评价维度得分均值与年代呈正相关(P<0.05),相关系数为0.13~0.54,年代可解释其6%~26%的变异;女护士和非新护士自我反思维度得分均值与年代呈负相关(P<0.05),相关系数分别为-0.29、-0.15,年代可分别解释其7%、10%的变异,而亚群体其他维度与年代不相关(P>0.05)。结论8年来我国临床护士职业认同的整体水平随年代进程有所改善,并存在性别、工龄、科室之间的差异;而女护士和非新护士自我反思能力历年来有所下降。Objective To explore the change of the professional identity of the clinical nurses with the age since 2010.Methods A cross-temporal meta-analysis that researched the professional identity of the clinical nurses was performed on 50 articles from 2011 to 2018 that used the professional identity of nurse quantitative list.Results There were significant correlations between the mean score of professional identity,professional evaluation,professional social skill,professional self-reflection and age(P<0.05),whose correlation coefficients were 0.13,0.22,0.14 and 0.14,respectively.The total average score of professional identity increased by 0.41,and that of cognitive evaluation,social skills and self-reflection increased by 17%,16%and 12%,respectively.The dimensions of social support and frustration response were not correlated with age(P>0.05).The total average score of subpopulations,social support dimension of male,female and new nurses,and cognitive evaluation dimension of ICU nurses were positively correlated with age(P<0.05),whose correlation coefficients were 0.13-0.54,and age could explain 6%-26%of its variation.The self-reflection of female nurse and not-new nurse was negatively correlated with age(P<0.05),whose correlation coefficient were-0.29 and-0.15,and age could explain 7%and 10%of its variation,respectively.Other dimensions of subpopulation were not correlated with age(P>0.05).Conclusion The professional identity of clinical nurse is significantly improved in the last 7 years,and there are differences among gender,working years and department.However,the professional self-reflection of female nurse and a non-new nurse is decline.
