摘　　要：目的探讨围生期妇女妊娠晚期B族溶血性链球菌(B group hemolytic streptococcus,GBS)带菌状态及GBS感染对妊娠结局的影响,为临床GBS预防和检测提供依据。方法采用显色平板法对4454例围产期妇女阴道及肛门直肠拭子进行GBS检测,并结合临床感染组和对照组患者实际妊娠结局进行综合分析。结果4454例患者中,GBS阳性率为9.85%(439/4454);大龄产妇,支原体感染,高血压等与GBS感染具有明显相关性。GBS阳性组胎膜早破、胎儿窘迫、羊水污染、早产、新生儿窘迫、新生儿肺炎、新生儿败血症、高胆红素血症发生率均较阴性组高。结论GBS感染可引发胎膜早破、胎儿窘迫、羊水污染、早产、新生儿败血症、新生儿高胆红素血症等不良妊娠结局,重视围生期孕妇产前GBS筛查,治疗是预防围生期妇女不良妊娠的必要措施。Objective To investigate the carrying status of B group hemolytic streptococcus(GBS) in perinatal women and the effect of GBS infection on pregnancy outcome, so as to provide evidence for the clinical prevention and detection of GBS. Methods A total of 4 454 vaginal and anal rectal swabs from perinatal women were conducted for GBS detection by chromogenic plate method. A comprehensive analysis was performed according to the actual pregnancy outcomes of the clinical infection group and the control group. Results Among the 4 454 patients, the positive rate of GBS was 9.85%(439/4 454);older women, mycoplasma infection, hypertension and GBS infection were significantly associated. Maternal premature rupture of membranes, fetal distress, amniotic fluid contamination, premature delivery, neonatal sepsis, neonatal hyperbilirubinemia in the infection group were higher than that in the control group. Conclusion GBS infection can cause premature rupture of membranes, fetal distress, amniotic fluid pollution, premature delivery, neonatal sepsis, neonatal hyperbilirubinemia and other adverse pregnancy outcomes. Prenatal GBS screening and treatment is a necessary measure for pregnant women to prevent adverse pregnancy.
