新生儿狼疮综合征10例临床分析

摘　　要：目的:探讨新生儿狼疮综合征的临床表现及转归,提高对新生儿狼疮综合征的认识。方法:回顾性分析2014年1月至2017年9月西安市儿童医院新生儿科住院的10例新生儿狼疮综合征的临床表现、辅助检查、转归及其母亲的相关疾病情况。结果:10例新生儿狼疮综合征中男5例,女5例;10例均出现皮肤损害,4例肝脏损害,1例先天性心脏传导束发育异常,1例血液系统损害。患儿均有抗核抗体阳性和(或)抗SSA抗体和(或)抗SSB抗体阳性。有2例母亲产前诊断为系统性红斑狼疮,其余母亲均为无症状,仅有抗核抗体阳性和抗SSA抗体和或抗SSB抗体阳性。除1例放弃治疗后失访,其余患儿均随访10 d^3年,患儿皮疹及其他症状均随自身抗体消耗而消失。结论:新生儿狼疮综合征临床主要以一过性皮疹为主要表现,可伴有肝脏、血液系统及先天性心脏传导束发育异常,其症状可随着自身抗体消失而自愈。母亲以系统性红斑狼疮及无症状的自身抗体阳性多见。Objective: To probe into the clinical manifestation and outcome of neonatal lupus erythematous( NLE),so as to improve the understanding. Methods: The clinical manifestation,auxiliary examinations,outcome in 10 children with NLE and related diseases of the mother in the neonatology department of Xi’an Children’s Hospital from Jan. 2014 to Sept. 2017 were retrospectively analyzed.Results: Of the 10 children with NLE,5 cases were male and 5 were female. Skin lesions were found in 10 cases,liver lesions in 4 cases,congenital abnormal development of cardiac conduction tract in 1 case,and blood system lesions in 1 case. All children were positive for anti-nuclear antibodies and( or) anti-SSA antibodies and( or) anti-SSB antibodies. Two of the mothers were diagnosed with systemic lupus erythematosus before birth,while the rest of the mothers were asymptomatic,with only positive for anti-nuclear antibodies and anti-SSA antibodies and anti-SSB antibodies. Except for one patient who was abandoned after treatment,all patients were followed up for 10 d to 3 years,the rash and other symptoms disappeared with the consumption of autoantibodies. Conclusion: The clinical manifestation of NLE is mainly transient rash,which may be accompanied by abnormal development of liver,blood system and congenital heart conduction tract. Its symptoms can be self-healing with the disappearance of autoantibodies. The mother’s symptoms are more common with lupus erythematosus and asymptomatic autoantibodies.
