解毒益元汤联合紫杉醇对老年子宫内膜癌患者术后免疫功能及康复水平的影响

摘　　要：目的探究解毒益元汤联合紫杉醇对老年子宫内膜癌术后免疫功能及康复水平的影响。方法选取2012年7月-2018年5月该院妇科诊治的104例老年子宫内膜癌术后患者为研究对象,按照随机数字表法分为观察组和对照组,每组各52例。对照组采用紫杉醇治疗,观察组在对照组基础上加用解毒益元汤。治疗9周,观察两组研究对象中医证候积分、临床疗效、免疫功能、糖类抗原125(CA125)水平、附睾蛋白4(HE4)水平及不良反应。结果治疗后,观察组阴道出血淋漓、神疲乏力、气短懒言、腰膝酸软、小腹冷痛证候积分低于对照组,差异有统计学意义(P<0.05)。观察组治疗总有效率明显高于对照组,差异有统计学意义(P<0.05)。观察组Ig G、Ig A水平低于对照组,NK、CD4+水平高于对照组,差异有统计学意义(P<0.05)。观察组CA125、HE4水平均低于对照组,差异有统计学意义(P<0.05)。观察组消化道反应、骨髓抑制发生率明显低于对照组,差异有统计学意义(P<0.05)。结论解毒益元汤联合紫杉醇治疗老年子宫内膜癌术后疗效显著,有助于缓解临床症状,改善其免疫功能,降低CA125、HE4水平,且不良反应较小。Objective To investigate the effect of Jiedu Yiyuan Decoction combined with paclitaxel on postoperative immune function and rehabilitation in elderly patients with endometrial cancer.Methods A total of 104 elderly patients with endometrial cancer who underwent gynecological diagnosis and treatment in the Second Hospital of Tianjin Medical University from July 2012 to May 2018 were enrolled and divided into observation group and control group according to the random number table method,with 52 cases in each group.The control group was treated with paclitaxel,and the observation group was supplemented with Jiedu Yiyuan Decoction on the basis of the control group.After 9 weeks of treatment,the TCM syndrome scores,clinical efficacy,immune function,carbohydrate antigen 125(CA125)humanepididymal protein 4(HE4)levels,and adverse reactions were observed.Results After treatment,the observation group had lower vaginal bleeding,Shenpi fatigue,shortness of breath and laziness to speak,soreness and weakness of waist and knees,and lower abdominal cold pain syndrome scores than the control group(P<0.05).The total effective rate of the observation group was significantly higher than that of the control group(P<0.05).The levels of Ig G and Ig A in the observation group were lower than those in the control group,while the NK and CD4+were higher than the control group(P<0.05).The levels of CA125 and HE4 in the observation group were lower than those in the control group(P<0.05).The incidence of digestive tract reaction and myelosuppression in the observation group was significantly lower than that in the control group(P<0.05).Conclusion Jiedu Yiyuan Decoction combined with paclitaxel is effective in the senile endometrial cancer,which can relieve clinical symptoms,improve its immune function,reduce the levels of CA125 and HE4 withless adverse reaction.
