孕早期彩色多普勒超声测量颈项透明层在胎儿畸形筛查中的价值

摘　　要：目的:探究孕早期彩色多普勒超声测量颈项透明层(NT)筛查胎儿畸形的应用价值。方法:选取2016年3月至2018年2月在阳江市阳东区妇幼保健院接受孕期检查的2810例孕1113+6周的孕妇作为研究对象,采用彩色多普勒超声检测所有孕妇胎儿的NT值,并进行胎儿畸形筛查。结果:2810例胎儿中,畸形胎儿一共55例。NT值<3.0 mm的共2735例,畸形胎儿占0.4%;NT值≥3.0 mm的胎儿共有75例,畸形胎儿占58.7%;随着NT值的增加,灵敏度和阴性预测值基本呈现下降的趋势,特异度和阳性预测值呈升高趋势;当NT值在3.03.4 mm时,其灵敏度为89.94%,特异度为95.76%,较为平衡。结论:在孕1113+6周妇女中采用彩色多普勒超声测量NT值筛查胎儿畸形的准确度较高,并且应该将NT值≥3.0 mm作为异常增厚的指标。Objective To explore the value of color Doppler ultrasound in the screening of fetal malformations in the early stage of pregnancy. Methods 2810 pregnant women who were pregnant at 11 ~ 13+6 weeks in Yangjiang Yangdong District Maternal and Child Health Hospital were selected as the research objects.Color Doppler ultrasound was used to detect the nuchal translucency of all pregnant women, and fetal malformations were screened. Results Of the 2810 fetuses, 55 were malformed. 2735 cases of NT < 3.0 mm, 0.4 % of deformed fetuses. There were 75 cases of fetus with NT value ≥ 3.0 mm, and 58.7% of malformed fetuses. With the increase of NT value, the sensitivity and negative predictive value basically showed a downward trend, and the specificity and positive predictive value showed an increasing trend. When the NT value is between 3.0 and 3.4 mm, the sensitivity is 89.94 % and the specificity is 95.76 %, which is relatively balanced. Conclusion The accuracy of color Doppler measurement of NT value in screening fetal malformations was higher in women with 11~13 weeks of pregnancy, and NT ≥ 3.0 should be an indicator of abnormal thickening.
