摘　　要：目的:探讨高强度聚焦超声(HIFU)治疗子宫腺肌瘤患者缓解痛经的可行性、安全性和有效性。方法:回顾性分析2016年3月至2017年3月广东省第二人民医院妇科临床诊断为子宫腺肌瘤导致痛经的36例患者,采用HIFU治疗,观察患者治疗前后疼痛评分、瘤体变化等指标,评估治疗效果。结果:治疗后3、6个月,月经干净后进行随访,对痛经症状进行评分、评级。痛经分级、评分均显著下降,差异具有统计学意义(P <0.05)。治疗前经B超和MRI测量腺肌瘤大小,HIFU治疗后第3、6个月经后复查B超或MRI,病灶区域与治疗前比较,差异具有统计学意义(P <0.05)。结论:HIFU治疗子宫腺肌瘤能保留完整的器官,对痛经的治疗效果好,治疗过程无痛苦、快捷方便、可重复治疗。Objective To explore the feasibility, safty, validity on alleviation of dysmenorrhea in HIFU treatment of endometrioma. Method Retrospective analysis on 36 cases clinically diagnosed as dysmenorrhea inducing by endometrioma from March 2016 to March 2017 in the second people’s hospital of Guangdong, and performance evaluation through chronic pain grade questionnaire after HIFU treatment. Results After 3 and 6 months of treatment, the menstruation was cleaned and followed up to score and rate the symptoms of dysmenorrhea. The dysmenorrhea grade and score were significantly decreased, and the difference was statistically significant(P < 0.05). The size of adenoma was measured by B-ultrasound and MRI before treatment. B-ultrasound or MRI was performed after 3 and 6 months after HIFU treatment. The lesions were recorded and the difference was statistically significant(P < 0.05). Conclusion HIFU treatment of endometrioma can preserve intact organ, and is an effective method of treating dysmenorrhea induced by it. And the process is indolent, efficient, convenient and repeatable.
