联合用药肠道准备法在功能性便秘的老年患者结肠镜检查前肠道准备的临床研究

摘　　要：目的:探讨小麦纤维素颗粒联合复方聚乙二醇电解质散用于功能性便秘的老年患者结肠镜检查前肠道准备的效果。方法:100例拟行结肠镜检查的慢性老年便秘患者,随机分为研究组(小麦纤维素颗粒联合复方聚乙二醇电解质散)和对照组(单用复方聚乙二醇电解质散),各50例。观察并比较两组波士顿肠道评分量表(BBPS)、结肠镜进镜时间及退镜时间、腺瘤检出率和肠道准备不良反应发生率。结果:研究组肠道准备评分[(7.21±1.05)分]和肠道准备有效率(96%)均显著优于对照组[(6.03±1.14)分,78%](P<0.05)。研究组的进镜时间[(5.5±3.2)min]较对照组[(6.1±3.7)min]显著缩短(P<0.05)。研究组的结肠腺瘤检出率较对照组高,然而差异无统计学意义(P>0.05)。研究组的肠道准备不良反应发生率(4%)显著低于对照组(32%)(P<0.05)。结论:功能性便秘的老年患者结肠镜检查前肠道准备使用小麦纤维素颗粒联合复方聚乙二醇电解质散,具有增加清洁度、提高镜检效果、增强患者耐受性和安全可靠等优点,适合临床推广使用。Objective:To evaluate the effect of modified bowel preparation Polyethylene Glycol Electrolytes Powder(SF-PEG,Shu Taiqing)in combination with Testa triticum tricum pufif on old patients with functional constipation before colonscopy.Methods:100 cases of old patients with functional constipation were randomly divided into two groups:experimental group who received SF-PEG(3 L)and Testa triticum tricum pufif,50 cases;control group:received SF-PEG(438.4 g,4 L),50 cases.Boston Bowel Preparation Scale scores(BBPs)and the detection rate of colorectal adenoma were assessed.The time of colonoscopy and the tolerability was recorded.Results:All patients underwent completely the bowel preparation and colonoscopy.The BBPS of colonoscopy of experimental group was significantly higher than that of control group(7.21±1.05 VS 6.03±1.14,P<0.05).The bowel preparation efficiency of experimental group was significantly than that of control group(96%VS 78%,P<0.05).The duration of endoscopy in the experimental group was significantly shorter than that in the control group[(5.5±3.2)min VS(6.1±3.7)min,P<0.05].However,there was no statistically significant difference in the detection rate of colorectal adenoma between the two groups(38%VS 32%,P>0.05).The adsverse effects of bowel preparation in experimental group(4%)was significantly lower than that of control group(32%)(P<0.05).Conclusion:SF-PEG in combination with Testa triticum tricum pufif in old patients with functional constipation for colonoscopy can improve tolerance to bowel preparation,cleanliness and effects of examination.It’s safely to be recommended in clinical practice.
