摘  要:目的研究无创机械通气治疗对AECOPD合并左心衰竭患者通气反应及血流动力学的影响。方法将我院收治的76例AECOPD合并左心衰竭患者按照不同治疗方式分为对照组与观察组,各38例。对照组给予面罩吸氧,观察组给予无创机械通气。比较两组患者的临床疗效。结果观察组患者的治疗总有效率明显高于对照组(P<0.05)。治疗7 d后,两组患者的PaCO2、MAP、CPIS评分均降低,PaO2/FiO2、CO、LVEF、FEV1/FVC均升高,且观察组优于对照组(P<0.05)。结论无创机械通气治疗AECOPD合并左心衰竭可有效提高患者的通气功能,改善血流动力学指标及心肺功能,减轻肺部感染,值得临床推广应用。Objective To study the effects of noninvasive mechanical ventilation treatment on ventilatory response and hemodynamics of patients with AECOPD and left heart failure. Methods A total of 76 patients with AECOPD and left heart failure admitted in our hospital were divided into control group and observation group according to the different treatment modes, with 38 cases in each group. The control group was given oxygen mask, and the observation group was given noninvasive mechanical ventilation. The clinical effects were compared between the two groups. Results The total effective rate of treatment in the observation group was significantly higher than that in the control group(P<0.05). After 7 days of treatment, PaCO2, MAP and CPIS score decreased, PaO2/FiO2, CO, LVEF and FEV1/FVC increased in both groups, and those in the observation group were superior than the control group(P <0.05). Conclusion Noninvasive mechanical ventilation on AECOPD with left heart failure can effectively improve the ventilatory function, the hemodynamic indexes and cardiopulmonary function, and it can reduce pulmonary infection, which is worthy of clinical promotion and application.
