放血疗法结合局部按摩治疗急性乳腺炎的随机对照研究

摘　　要：目的:观察放血疗法结合局部按摩治疗急性乳腺炎的临床疗效。方法:将60例急性乳腺炎患者随机分为对照组和观察组,各30例。对照组予以局部按摩治疗,观察组在此基础上予放血治疗,疗程为1个月。治疗前后评定主要症状体征评分;测定静脉血白细胞计数、中性粒细胞计数及静脉血白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、干扰素-γ(IFN-γ)水平;并于治疗结束后评定治疗效果。结果:治疗后观察组总有效率明显高于对照组(P<0.05);观察组乳房疼痛、泌乳情况、乳房肿块、红肿面积评分及主要症状体征总分均明显低于对照组(均P<0.05);观察组白细胞计数及中性粒细胞计数下降较对照组更为明显(均P<0.05);治疗后观察组患者IL-6、TNF-α水平明显降低,IFN-γ水平则明显升高,且与对照组比较有统计学差异(均P<0.05)。结论:放血疗法结合局部按摩治疗急性乳腺炎疗效确切,能明显改善乳房疼痛、泌乳情况、乳房肿块、红肿面积等主要症状体征,恢复偏高的白细胞、中性粒细胞计数水平。这可能与其下调IL-6及TNF-α水平、提升IFN-γ水平有关。Objective:To observe the clinical effect of blood-letting therapy combined with local massage for acute mastitis.Methods:60 cases were randomly divided into the control group(n=30)and treatment group(n=30).The control group was treated with local massage;on which basis,the treatment group was also treated with blood-letting therapy.Both groups were treated for one month.Before and after the treatment,main symptoms score was evaluated.White blood cell count,neutrophil count,and the serum levels of interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)and interferon-γ(IFN-γ)were measured.After the treatment,therapeutic effect was assessed.Results:The effective rate in treatment group was higher than that in the control group(P<0.05).After the treatment,breast pain,lactation,breast lump,swelling area and total symptoms scores were significantly decreased in the treatment group than those in the control group(P<0.05).The white blood cell count and neutrophil count were lower after the treatment in treatment group than those in the control group(P<0.05).The serum levels of IL-6 and TNF-αwere decreased and the serum level of IFN-γwas increased in the treatment group(P<0.05);of which the differences were significant between the two groups(P<0.05).Conclusion:Blood-letting therapy combined with local massage for acute mastitis shows obvious curative effect.It can relieve the clinical symptoms of breast pain,lactation,breast lump and swelling area,and decrease white blood cell count and neutrophil count.These may be related to the down-regulation of serum levels of IL-6 and TNF-αand the up-regulation of serum level of IFN-γ.
