银质针导热疗法治疗强直性脊柱炎研究进展

摘　　要：强直性脊柱炎(AS)是一种慢性自身免疫性疾病,以侵犯骶髂关节和中轴关节为主,临床上多表现为炎性腰背痛、僵硬与活动受限。本病多发于青壮年男性,并具有一定的家族遗传性。目前治疗方式包括:药物、骶髂关节注射、物理及手术治疗。银质针导热疗法是以宣氏银质针技术为基础,利用导热巡检仪加热,使其在深部组织能够达到精确、持续控温的作用。通过镇痛、解痉和改善微循环等机制来打破AS发病中“疼痛-受限-疼痛”的恶性循环,发挥良好的治疗作用,在改善关节活动度和缓解疼痛方面,其治疗效果要优于一般药物与物理治疗。在治疗AS时通常以骶髂关节、髋关节、脊柱腰椎段为主要针刺部位,虽然进针时部分患者针感强烈,但其安全性较高,治疗前后的生命体征无明显变化。本研究通过总结银质针导热疗法在强直性脊柱炎治疗中的临床研究,旨在为银质针导热疗法在强直性脊柱炎治疗中的进一步应用提供参考。Ankylosing spondylitis(AS)is a kind of autoimmune disease,it is manifested as chronic connective tissue disorder which mainly involves sacroiliac joint and axial joint.The typical symptoms are inflammatory pain of lower back,stiffness and limitation of mobility.This disease occurs mostly in the young males,which is prone to family inheritance.The current treatments of AS include medications,sacroiliac joint injection,physical and surgery therapy.Thermal silver needle therapy is based on Xuan's silver needle technique and heated by thermal conductivity detector achieving accurate and constant temperature control in deep tissue.Through analgesia,spasmolysis and improvement of microcirculation,it can break the vicious circle of“pain-limitation of motion-pain”in AS and play a good therapeutic role,and it is much better than standard medication and physiotherapy in improving the activity of joints and relieving pain.Needling points are mainly focused on sacroiliac joint,hip joint and lumbar spine in the treatment of AS,although some patients felt strong needle sensation at the time of needle insertion,the safety of needle insertion was high,and there was no significant changes in vital signs before and after treatment.This study summarized the clinical study of thermal silver needle therapy in treating AS,aiming to provide references for the further application of thermal silver needle therapy in the treatment of AS.
