耳穴贴压疗法干预青少年近视的Meta分析

摘　　要：目的系统评价耳穴贴压疗法干预青少年近视的临床疗效。方法计算机检索并筛选耳穴贴压防治青少年近视的病例对照研究文献,对文献中耳穴贴压治疗近视的有效率指标进行Meta分析,并对治疗前后近视患者的视力、屈光度及眼球生物学参数的变化进行分析。结果有效率指标Meta分析显示,耳穴贴压和非睫状肌麻痹类方法治疗青少年近视具有显著性优势[轻中度近视:OR=1.70,95%CI(1.35,2.14),P<0.01;假性近视:OR=7.10,95%CI(4.16,12.14),P<0.01];耳穴贴压和睫状肌麻痹剂治疗青少年近视同样具有显著性优势[轻中度近视:OR=3.68,95%CI(2.46,5.50),P<0.01;假性近视:OR=4.62,95%CI(2.70,7.93),P<0.01]。生物学参数分析显示耳穴贴压疗法干预青少年近视有利于改善视力、缓解屈光度进展,伴随眼轴增长速率下降、前房加深和眼压降低等变化趋势。结论耳穴贴压疗法具备有效的近视防治作用,且对于假性近视效果为佳,是现阶段值得推广、行之有效的防控近视策略之一。后续研究需更规范设计的临床试验来验证耳穴贴压疗法对眼球生物学参数的影响,并研究其潜在的生理作用机制。Objective To systematically evaluate the clinical efficacy of auricular acupressure therapy in the treatment of juvenile myopia.Methods A computer-based search and screening of case-control studies on the auricular acupressure in the prevention and treatment of juvenile myopia was conducted.Meta-analysis of the effective indices of auricular acupressure therapy for myopia in the literature was performed.The vision,diopter and eyeball biological parameters of the myopic patients before and after treatment were analyzed.Results It was found by the meta-analysis of the effective indices that auricular acupressure was significantly superior to non-ciliary muscle paralysis in the treatment of ju venile myopia[mild and moderate myopia:OR=1.70,95%CI(1.35,2.14),P<0.01;pseudomyopia:OR=7.10,95%CI(4.16,12.14),P<0.01],and that auricular acupressure was also significantly superior to ciliary muscle paralysis in the treatment of juvenile myopia[mild and moderate myopia:OR=3.68,95%CI(2.46,5.50),P<0.01;pseudomyopia:OR=4.62,95%CI(2.70,7.93),P<0.01].It was found by biological parameters analysis that auricular acupressure therapy intervention for juvenile myopia was beneficial to improve vision and alleviate diopter progression,with a decreased axial growth rate,deepened anterior chamber and decreased intraocular pressure.Conclusion Auricular acupressure therapy has effective myopia prevention and treatment effect,especially for pseudo-myopia,which is one of the myopia prevention and control strategies worthy of promotion and proven effective at this stage.Subsequent studies should design clinical trials that are more standardized in order to validate the influence of auricular acupressure therapy on the eyeball biological parameters and to study its underlying physiological mechanism.
