摘　　要：目的研究L市社区老年人的卫生服务利用情况,对不同社区进行综合评价,根据调查结果提出切实可行的对策和建议,为社区卫生服务中心针对老年人开展工作提供参考。方法对所调查老年人的基本情况进行描述性分析;采用logistic回归方法分析社区卫生服务利用的影响因素;采用TOPSIS法对不同社区卫生服务利用情况进行综合评价。结果被调查老年人的两周就诊率为29.7%,两周患者就诊率为68.5%,老年人过去一年住院率为28.3%;logistic回归分析结果显示,影响老年人两周就诊率和住院率的主要因素为是否患有慢性疾病(P<0.05);Topsis评价结果显示,经济水平和文化程度高的被调查者社区卫生服务利用情况相对较好。结论L市社区老年人卫生服务利用情况整体较好,但是不同社区之间存在差异,部分老年人认为社区医疗机构的医生技术能力较低。因此,要提高基层医务人员的能力,加快培养全科医生,从而提高社区卫生资源的利用。OBJECTIVE To find out the current situation of utilization in the elderly in the urban area of L City,conduct comprehensive evaluation of different communities,put forward feasible countermeasures and suggestionsaccording to the survey results,carry out the work for the elderly to provide a basis for the community health service centers.METHODS Descriptive analysis was conducted on the basic situation of the investigatedelderly.Logistic regression method was conducted to analyze the influence factors of community health service utilization;Topsis was used to evaluate the utilization of different community health services.RESULTS The two-week visiting rate was 29.7%,the two-week visiting rate was 68.5%,and the hospitalization rate in the past year was 28.3%.Logistic regression analysis showed that the main factor affecting the two week consultation rate and hospitalization rate of the elderly was whether to suffer from chronic diseases or not.Topsis evaluation showed that the utilization of health services was relatively good in some communities with high economic and educational level.CONCLUSION The utilization of health services in the elderly in the community of L city was good,but there were differences among different communities,some elderly people believed that the doctors’technical level was low in the community medical institutions.Therefore,it was necessary to improve the ability of medical staff at the grass-roots level and speed up the training of general practitioners so as to improve the utilization of community health resources.
