乌灵胶囊联合圣约翰草提取物片治疗脑梗死恢复期并抑郁临床研究  

Clinical Study on Wuling Capsules Combined with Extract of St. John’s Wort Tablets in the Treatment of Cerebral Infarction with Depression at the Convalescent Stage

摘　　要：目的探讨乌灵胶囊联合圣约翰草提取物片治疗脑梗死恢复期并抑郁的临床疗效。方法选取医院2016年1月至2018年5月收治的脑梗死恢复期并抑郁患者93例,按随机数字表法分为对照组(46例)和研究组(47例)。两组患者均予圣约翰草提取物片口服(每次1片,每日3次),研究组患者加服乌灵胶囊(每次3粒,每日3次),两组均连续治疗8周。结果研究组总有效率为93.62%,明显高于对照组的80.43%(χ^2=4.559,P=0.033<0.05);治疗后,两组患者汉密尔顿抑郁量表(HAMD)评分均明显低于治疗前,简易精神状态检查量表(MMSE)评分均明显高于治疗前,且研究组改善程度明显优于对照组(P<0.05);治疗后,两组患者血清去甲肾上腺素(NE)、多巴胺(DA)水平均较治疗前明显升高,血清肿瘤坏死因子-α(TNF-α)、白细胞介素6(IL-6)及白细胞介素23(IL-23)水平均较治疗前明显下降,且研究组改善程度明显优于对照组(P<0.05);研究组不良反应发生率与对照组相当(8.51%比6.52%,χ^2=0.132,P=0.716>0.05)。结论乌灵胶囊联合圣约翰草提取物片治疗脑梗死恢复期并抑郁的疗效确切,能明显改善临床症状和认知功能,其作用机制可能与抑制炎性反应有关。Objective To investigate the clinical efficacy of Wuling Capsules combined with Extract of St.John's Wort Tablets in the treatment of cerebral infarction with depression at the convalescent stage.Methods Totally 93 patients with cerebral infarction with depression at the convalescent stage admitted to our hospital from January 2016 to May 2018 were selected and divided into the control group(n=46)and the study group(n=47)according to the random number table method.The patients in the two groups took Extract of St.John's Wort Tablets orally(1 tablet each time,3 times a day),and the patients in the study group were treated with Wuling Capsules(3 capsules each time,3 times a day).Both groups were continuously treated for 8 weeks.Results The total effective rate of the study group was 93.62%,which was significantly higher than 80.43%of the control group(χ^2=4.559,P=0.033<0.05).After treatment,t he score of Hamilton Depression Scale(HAMD)in the two groups was significantly lower than that before treatment,the score of Minimum Mental State Examination(MMSE)in the two groups was significantly higher than that before treatment,and the improvement of the study group was significantly better than that of the control group(P<0.05).After treatment,the levels of serum norepinephrine(NE)and dopamine(DA)in the two groups were significantly higher than those before treatment,the levels of serum tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and interleukin-23(IL-23)in the two groups were significantly lower than those before treatment,and the improvement of the study group was significantly better than that of the control group(P<0.05).The incidence of adverse reactions in the study group was similar to that in the control group(8.51%vs.6.52%,χ^2=0.132,P=0.716>0.05).Conclusion Wuling Capsules combined with Extract of St.John's Wort Tablets are effective in the treatment of patients with cerebral infarction with depression at the convalescent stage,which can obviously improve the clinical symptoms and cognitive function,an
