摘　　要：目的探讨5-氨基酮戊酸光动力疗法(ALA-PDT)联合咪喹莫特治疗皮肤基底细胞癌(BCC)的疗效及对免疫功能的影响。方法选取医院2015年3月至2018年3月收治的BCC患者104例,采用随机数字表法分为对照组和观察组,各52例。两组患者均予ALA-PDT治疗,观察组患者加用咪喹莫特,两组均治疗12周。结果观察组的临床总有效率和创面美容效果优良率分别为88.46%和86.54%,显著高于对照组的76.92%和65.38%(P<0.05);与治疗前比较,两组患者治疗后的CD4^+、CD4^+/CD8^+、自然杀伤(NK)细胞和CD8^+水平显著降低,且观察组患者上述细胞改善程度显著优于对照组(P<0.05);观察组CD3^+水平显著升高(P<0.05);观察组和对照组的不良反应发生率相当(11.54%比13.46%,P>0.05)。结论ALA-PDT联合咪喹莫特治疗BCC有一定创面修复疗效和美容疗效,并可提高患者的免疫功能。Objective To investigate the efficacy of 5-aminolevulinic acid-photodynamic therapy(ALA-PDT)combined with imiquimod in the treatment of cutaneous basal cell carcinoma(BCC)and its effect on immune function.Methods Totally 104 patients with BCC admitted to our hospital from March 2015 to March 2018 were selected and randomly divided into the control group and the observation group,52 cases in each group.The patients in the two groups were treated with ALA-PDT,on this basis,the patients in the observation group were treated with imiquimod.Both groups were treated for 12 weeks.Results The total clinical effective rate and the excellent and good rate of cosmetic effect in the observation group were 88.46%and 86.54%,which were significantly higher than 76.92%and 65.38%in the control group(P<0.05).Compared with those before treatment,the levels of CD4^+,CD4^+/CD8^+,natural killer(NK)cells and CD8^+in the two groups were significantly decreased,and the improvement of the observation group was significantly better than that of the control group(P<0.05).The level of CD3^+in the observation group was significantly increased(P<0.05).The incidence of adverse reactions in the observation group and the control group was similar(11.54%vs.13.46%,P>0.05).Conclusion ALA-PDT combined with imiquimod in the patients with BCC has significant efficacy of wound repair and cosmetic effect,and it also can effectively improve the immune function of patients.
