医共体背景下铜仁地区县级及以下医疗机构新农合病人就诊情况分析

摘　　要：目的:分析贵州省铜仁地区县域医共体组建后县级及以下医疗机构新农合病人就诊及基金流向。方法:选取基本情况相似的松桃县(2017年实施医共体政策)、思南县和沿河县(2018年实施医共体政策)进行普查,同比、环比医共体组建后各县县级及以下医疗机构门诊人次增幅、住院人次增幅及新农合基金增幅情况。结果:同比2017年1-6月、2018年1-6月松桃县、思南县和沿河县县外医疗机构门诊人次增幅为-99.46%、266.03%和1671.15%,住院人次增幅为194.57%、-13.15%和21.1%,基金增幅为117.17%、-9.79%和3.39%;县级医疗机构门诊人次增幅为185.69%、28.84%和15.9%,住院人次增幅为-9.5%、28.22%和132.9%,基金增幅为-25.89%、51.45%和184.62%;环比2017年7-12月,2018年1-6月松桃县、思南县和沿河县县外医疗机构门诊人次增幅为-99.43%、22.8%和96.38%,基金增幅为-42.38%、-9.79%和61.9%。结论:医共体组建后,各县门诊人次和住院人次有不同程度的上升,基金也有向县内回流的趋势。Objective:To analyze the impact of medical alliance on the flow of NCMS Patients in county hospitals and hospitals below county level in Tongren Area in Guizhou province.Methods:The survey was performed in the counties with similar situation:Songtao County(medical alliance starting in 2017),Sinan County and Yanhe County(medical alliance starting in 2018).The changes in the number of outpatient visits,the number of inpatients and the amount of medical fund before and after medical alliance were compared in the counties and between the counties.Results:Compared the period from January to June 2018 with the period from January to June of 2017 in the medical institutions of Songtao County,Sinan County and Yanhe County,the change rates of the number of outpatient visits were-99.46%,266.03%and 1671.15%;the change rates of the number of inpatients were 194.57%,-13.15%and 21.1%;the change rates of the medical fund were increased by 117.17%,-9.79%and 3.39%;in county-level medical institutions,the numbers of outpatients were increased by 185.69%,28.84%and 15.9%,and the change rates of the number of inpatients were-9.5%,28.22%and 132.9%.The medical fund was increased by-25.89%、51.45%and 184.62%.Compared the period from January to June 2018 with the period from July to December of 2017,the change rates of the number of outpatient visits were-99.43%,22.8%and 96.38%,and the medical fund growth rate was-42.38%,-9.79%and 61.9%.Conclusion:The medical alliance not only increased the number of outpatients and inpatients in each county,but also attracted the medical fund back to the county.
