摘 要:目的比较采用内轴型(MP)与后稳定型(PS)膝关节假体行全膝关节置换术(TKA)后下肢负重位全长X线片的差异。方法将行TKA的68例患者(98膝)随机分为MP组(34例48膝)与PS组(34例50膝)。术前、术后行下肢负重位全长X线片检查,对比分析术前、术后两组X线片并测量髋膝踝角(HKAA)及膝关节面角(FTFA)变化。结果患者均获得随访,时间3~6(4.2±1.6)个月。MP组患者HKAA术前为172.3°±7.6°,术后为176.8°±2.9°,术后较术前矫正6.8°±3.9°(P<0.05)。PS组患者HKAA术前为174.1°±9.3°,术后为177.0°±4.3°,术后较术前矫正7.4°±4.9°(P<0.05)。MP组与PS组FTFA分别由术前5.7°±3.1°、5.5°±3.0°均矫正为术后0°(P<0.05)。术前、术后两组间HKAA、FTFA对比差异无统计学意义(P>0.05)。结论MP与PS膝关节假体系统均能形成良好力线,达到矫正膝关节畸形的目的,两组X线片对比无明显差异。Objective To compare the difference of lower extremity weight-bearing full-length X-ray radiographs between medial pivot(MP)and posterior cruciate-stabilizing(PS)knee prosthesis after total knee arthroplasty(TKA).Methods The 68 TKA patients(98 knees)were randomly divided into the MP group(34 cases 48 knees)and PS group(34 cases 50 knees).The weight bearing full length X-ray radiographs of the lower extremities were taken preoperatively and postoperatively.The changes of femorotibial facet angle(FTFA)and hip-knee-ankle angle(HKAA)were compared and analyzed.Results All cases were follewed up for 3~6(4.2±1.6)months.The preoperative HKAA was 172.3°±7.6°,which was increased to 176.8°±2.9°postoperatively,with a correction of 6.8°±3.9°(P<0.05)in the MP group.The preoperative HKAA was 174.1°±9.3°,which was increased to 177.0°±4.3°postoperatively,with a correction of 7.4°±4.9°(P<0.05)in the PS group.The preoperative FTFA of MP group and PS group were 5.7°±3.1°and 5.5°±3.0°,respectively,which all were corrected as 0°(P<0.05).There were no statistically significant differences between the two groups of HKAA and FTFA(P>0.05).Conclusions MP and PS knee prosthesis system can form a good line of force,achieve the purpose of correcting knee joint deformity.And there is no significant difference between the X-ray of MP and PS knee prosthesis system.
