摘　　要：目的 分析影响儿童股骨骨折弹性钉固定后患肢过度生长的因素。方法 将164例行弹性钉固定的股骨骨折患儿按术后2年患儿患肢过度生长情况分为<1 cm组(n=122)与≥1 cm组(n=42)。采用logistic回归分析筛选影响儿童股骨骨折弹性钉固定术后患肢过度生长的因素。结果 ≥1 cm组性别、年龄、体重、骨折部位、致伤原因、骨折分型、骨折愈合时间与<1 cm组比较差异无统计学意义(P>0.05);≥1 cm组不稳定骨折比例、合并脑损伤比例均高于<1 cm组(P<0.05)。多元logistic回归分析结果发现,骨折类型为影响儿童股骨骨折弹性钉固定术后患肢过度生长的独立危险因素(P<0.05)。结论 骨折类型为影响儿童股骨骨折弹性钉固定术后患肢过度生长的相关因素,不稳定股骨骨折患儿术后患肢过度生长程度更高。Objective To analyze factors affecting excessive growth of affected limbs in children with femoral fractures after elastic nail fixation.Methods A total of 164 children patients with femoral fractures who were treated with elastic nail fixation, were divided into <1 cm group (n=122) and ≥1 cm group (n=42),according to the situation of excessive growth of affected limbs at 2 years after operation.The logistic regression analysis was used to screen out the factors affecting excessive growth of affected limbs in children with femoral fractures after elastic nail internal fixation.Results There were no significant differences between ≥1 cm group and <1 cm group in the gender,age,weight,fracture site,cause of injury,fracture type and fracture healing time (P>0.05).The rate of unstable fractures and combined brain injury in ≥1 cm group were higher than those in <1 cm group (P<0.05).Multivariate logistic regression analysis results showed that fracture type was an independent risk factor for excessive growth of affected limbs after elastic nail fixation (P<0.05).Conclusions Fracture type is a related factor affecting the excessive growth of affected limbs after elastic nail fixation in children with femoral fractures.The degree of excessive growth of affected limbs is higher in children patients with unstable femoral fractures.
