摘　　要：目的探讨新兵训练胸部撞击伤高分辨率CT一站式检查的临床价值。方法总结分析86例新兵训练胸部撞击伤的CT资料,均行高分辨率CT平扫和胸廓骨三维重建。结果高分辨率CT一站式检查能清晰显示胸部损伤情况:胸壁软组织肿胀78例,伴肺挫裂伤46例(内有肺实变18例),气胸14例,胸腔积液24例,血气胸6例,纵膈积气3例,纵膈血肿2例,肋骨骨折28例,肋软骨骨折2例,胸椎椎体及附件骨折3例,肝脏挫裂伤2例,脾脏挫裂伤1例。胸部CT阴性8例。术前CT诊断与手术结果及临床随访结果一致。结论新兵训练胸部撞击伤行CT一站式检查,可以减少患者的搬动,且能够明确胸部损伤的部位、范围及性质,对损伤的诊断及预后判断具有决定性的作用,在临床上有很高的实用价值。Objective To explore the clinical value of one-stop high resolution CT examination in the training-caused chest impact in new recruits. Methods CT data about chest impact injury of 86 new recruits caused during training were summarized and analyzed, high resolution non-enhanced CT scan was performed and three-dimensional reconstruction was applied to construct the thoracic bone model. Results Chest injury was clearly displayed during one-stop high resolution CT examination: 78 cases of soft tissue swelling on chest wall, among whom 46 cases were companied by pulmonary contusion(including 18 cases of lung consolidation), 14 cases by pneumothorax, 24 by pleural effusion, six by hemopneumothorax, three by mediastinal emphysema, two by mediastinal hematoma, 28 by rib fracture, two by costal cartilage fracture, three by fracture of thoracic vertebral body and adnexa, two by liver contusion, and one by spleen contusion and 8 cases of negative chest CT. Preoperative CT diagnosis result was consistent with surgical result and clinical follow-up result. Conclusion Performing one-stop CT examination in chest impact for new recruits can avoid unnecessary movement, precisely locate the chest injury, and determine the size and nature, playing a decisive role in the injury diagnosis and prognosis, hence, it is highly practical in clinical field.
