手把手小班培训在输尿管软镜培训中的应用

摘　　要：目的探讨手把手小班输尿管软镜培训教学对于短期培训效果的价值。方法2017年1月—2018年9月期间举办了10期手把手小班培训,学员共120名,培训开始前、后对所有医师进行问卷调查,并记录学员培训前、后找到肾盂及所有目标肾盏的时间,培训前、后漏盏率和模拟操作情况主观评分。结果培训后操作时间较培训前明显缩短,漏盏率明显下降、GRS明显提高,差异均有统计学意义(P均<0.05);问卷调查结果显示114人(98.3%)认为规范化系统输尿管软镜培训对于输尿管软镜临床能力的提高有很大帮助。42人(36.2%)3个月后独立开展手术。结论手把手小班“理论学习+模拟练习+手术体验+后期指导”的“4+”培训教学可以明显缩短输尿管软镜学习曲线,有利于快速掌握输尿管软镜操作技巧,取得较好的教学效果,可作为技术难度较高的培训模式进行应用。Objective To evaluate the value of the“teach by doing”mode in the training of flexible ureteroscopy.Methods 120 trainees in the flexible ureteroscopy training class in our hospital from January 2017 to September 2018,in were prospectively selected.Questionnaire survey was conducted among all trainees before and after the training,and the time of finding renal pelvis and all target renal calyces before and after the training was recorded,as well as the missed calyces rate and the subjective score of global rating scale(GRS)before and after the training and simulated operation.Results After the training,the operating time was significantly shorter than before the training,the missed calyces rate was significantly reduced,and the GRS was significantly increased,with statistically significant differences.The results of the questionnaire survey showed that 114(98.3%)people believed that the standardized system of flexible ureteroscopy training was of great help to improve the clinical ability of flexible ureteroscopy.Forty-two patients(36.2%)performed the operation independently after 3 months.Conclusion The"4+"training and teaching of"theoretical learning+simulation practice+surgical experience+post-guidance"in“teach by doing”mode training can significantly shorten the learning curve of ureteroscope,which is conducive to quickly mastering the operation skills of ureteroscope and achieving better teaching effect.Therefore,it can be promoted and applied as a training mode with high technical difficulty.
