单侧、双侧鼻腔鼻窦病变临床特征的比较分析

摘　　要：目的探讨和总结单、双侧鼻腔鼻窦病变的规律和特点。方法将单、双侧鼻腔鼻窦病变患者各100例分为单、双侧组并对两组临床资料进行回顾,对CT及病理进行着重分析。结果单、双侧鼻腔鼻窦病变的临床症状多为鼻塞、流涕;病程分别为2.4、5.1年。术前CT诊断正确率分别为84%(单侧组)、86%(双侧组)。单侧组术后病理确诊为普通炎症性病变53例,非普通炎症性病变47例。双侧组术后病理确诊为普通炎症性病变79例、非普通炎症性病变21例。单侧组的病程短于双侧组(P<0.05)、非普通炎症性病变多于双侧组(P<0.05),两组的CT诊断正确率无明显差别(P>0.05)。结论单、双侧鼻腔鼻窦病变临床症状均无特异性;CT扫描对单、双侧病变都有重要意义;单侧鼻腔鼻窦病变近半数为非普通炎症性病变、病程发展相对较快,对此应加以注意。Objective To investigate the clinical characteristics of unilateral, bilateral nasal cavity and paranasal sinuses lesions.Methods Patients with unilateral, bilateral nasal cavity and paranasal sinus lesions were divided into unilateral group(100 patients)and bilateral group(100 patients). The clinical data of the two groups were retrospectively analyzed. Results The clinical symptoms of unilateral and bilateral nasal cavity and paranasal sinuses lesions mostly included nasal obstruction and rhinorrhea. The average duration of unilateral and bilateral group was 2.4 and 5.1 years respectively, and the accuracy of CT diagnosis was 84% and 86%respectively. In unilateral groups, 53 cases were diagnosed as common inflammatory lesions, 47 cases as non-common inflammatory lesions. In bilateral groups, 79 cases were diagnosed as common inflammatory lesions, 21 cases as non-common inflammatory lesions.The duration of disease in unilateral group was significantly shorter than that in bilateral group(P<0.05), with significantly more cases of non-common inflammatory lesions(P<0.05). There was no significant difference of diagnostic accuracy of CT between the two groups(P>0.05). Conclusion No obvious difference of clinical symptoms is found between unilateral and bilateral nasal cavity and paranasal sinus lesions. CT scan is of great significance. Nearly half of unilateral nasal cavity and paranasal sinus lesions are characterized as non-common inflammatory lesions, with relatively fast course of disease, which requires extensive attention.
