单侧开颅大脑镰切开术与双侧骨窗开颅术治疗双额叶脑挫裂伤的效果对比

摘　　要：目的比较单侧开颅大脑镰切开术与双侧骨窗开颅术治疗双额叶脑挫裂伤的临床效果。方法选择2016年1月至2018年12月我院收治的双额叶脑挫裂伤患者42例,按照治疗方法的不同分为对照组(n=17)和观察组(n=25)。对照组患者给予双侧骨窗开颅术治疗,观察组患者给予单侧开颅大脑镰切开术治疗。比较两组的手术相关指标以及手术30 d后的临床疗效。结果观察组的手术时间、术后意识恢复时间、卧床时间、总住院时间均短于对照组,术中出血量少于对照组(P<0.05)。手术30 d后,观察组的治疗总有效率为92.00%,显著高于对照组的64.71%(P<0.05)。结论与双侧骨窗开颅术相比,单侧开颅大脑镰切开术治疗双额叶脑挫裂伤的临床效果更佳,可有效缩短患者病程,提高临床疗效。Objective To compare the clinical effects of unilateral craniotomy via cerebral falx incision approach and bilateral bone window craniotomy in the treatment of bilateral frontal lobe brain contusion. Methods 42 patients with bilateral frontal lobe brain contusion admitted to our hospital from January 2016 to December 2018 were selected and divided into two groups according to the treatment methods. 17 cases of the control group were treated with bilateral bone window craniotomy, and 25 cases of the observation group were treated with unilateral craniotomy via cerebral falx incision approach. The surgery related indicators, and the clinical efficacy at 30 d after surgery were compared between two groups. Results The operation time, postoperative consciousness recovery time, bed rest time and total hospitalization time of the observation group were shorter than those of the control group, and the amount of intraoperative blood loss was less than that of the control group(P<0.05). At 30 d after surgery, the total effective rate of the observation group was 92.00%, significantly higher than 64.71% of the control group(P<0.05). Conclusions Compared with bilateral bone window craniotomy, unilateral craniotomy via cerebral falx incision approach has better clinical effect in the treatment of bilateral frontal lobe brain contusion, which can effectively shorten the course of disease, and improve the clinical efficacy.
