中西医结合保守治疗血β-HCG值2000～5000 IU/L输卵管妊娠的疗效评价

摘　　要：目的:观察中西药结合保守治疗血β-HCG值2000~5000 IU/L的异位妊娠临床疗效。方法:回顾性分析2014年1月至2018年9月西南医科大学附属医院妇产科血β-HCG值在2000~5000IU/L之间的60例异位妊娠患者,观察组和对照组各30例。对照组予以口服米非司酮片50 mg,Bid;给药当天同时予以甲氨蝶呤(MTX)臀部肌肉注射;3 d为1疗程,可重复使用。观察组在其基础上加服中药汤剂桃红四物汤加减,辩证施治,1剂/d,水煎后服用,5 d为1疗程,可重复使用。结果:观察组治愈率为93.3%;对照组治愈率为73.3%,差异具有统计学意义(χ^2=4.320,P=0.04)。与对照组比较,观察组血β-HCG值恢复正常时间、盆腔包块消失时间及住院天数均明显缩短,差异均具有统计学意义(P <0.01)。观察组不良反应发生率(20%)明显低于对照组(50%),差异具有统计学意义(χ^2=4.934,P=0.015)。结论:中西医结合保守治疗血β-HCG值2000~5000IU/L的异位妊娠治愈率较高,且能缩短住院天数,值得推广使用。Objective:To investigate the clinical efficacy of conservative integrated traditional Chinese andWestern medicine therapy for ectopic pregnancy with blood beta-human chorionic gonadotropin(β-HCG)levels be-tween 2000 and 5000 IU/L.Methods:A retrospective analysis was performed on 60 cases of ectopic pregnancywith bloodβ-HCG levels between 2000 and 5000 IU/L from the Department of Obstetrics and Gynecology,The Af-filiated Hospital of Southwest Medical University,from January 2014 to September 2018.They were randomly divid-ed into observation group and control group,with 30 cases in each group.The control group was given oral mifepris-tone tablets at a dose of 50 mg twice daily and injection of methotrexate on the day of mifepristone administration for3 days per course of treatment,allowing for multiple courses.In addition to the treatment for the control group,theobservation group received the traditional Chinese medicine decoction Taohong Siwu decoction,which was modifiedaccording to syndrome differentiation.The decoction was orally administered 1 dose daily for 5 days per course oftreatment,allowing for multiple courses.Results:The cure rates of the observation and control groups were 93.3%and 73.3%,respectively,with a significant difference between the two groups(χ^2=4.320,P=0.04).The observationgroup had significantly shorter time to normalization of bloodβ-HCG levels,time to disappearance of pelvic masses,and length of hospitalization than the control group(P<0.01).The incidence of adverse reactions in the observationgroup was significantly lower than that in the control group(20%vs 50%,χ^2=4.934,P=0.015).Conclusion:Con-servative integrated traditional Chinese and Western medicine therapy has a high cure rate for ectopic pregnancywith bloodβ-HCG levels of 2000~5000 IU/L,and shortens the length of hospitalization,deserving wide application.
