鼓膜切开加正行吹张治疗儿童腺样体肥大并发分泌性中耳炎的临床分析

摘　　要：目的探讨鼓膜切开加正行吹张治疗腺样体肥大并发分泌性中耳炎(AH并发SOM)患儿的临床效果。方法选择2017年1月至2018年4月在本院就诊并确诊为AH并发SOM的60例患儿为研究对象,随机分为对照组和观察组,各30例。两组患儿均给予腺样体切除术+鼓膜切开术,观察组患儿加行正行吹张术,比较两组患儿的临床治疗效果。结果观察组患儿术后2周、3个月和6个月时整体治疗有效率(83.67%、87.76%和93.88%)略高于对照组(76.69%、82.98%和87.23%),差异无统计学意义;观察组患儿术后2周、3个月和6个月时治疗有效率(75.76%、81.82%和90.91%)均高于对照组(66.67%、73.33%和80.00%),组间差异有统计学意义(P<0.05)。结论鼓膜切开后联合正行吹张治疗AH并发SOM患儿临床效果确切,尤其在治疗黏液性中耳积液患儿方面效果显著。Objective To discuss the effect of the treatment of adenoidal hypertrophy(AH)complicated with secretory otitis media(SOM)in children by the puncture of the ear drum and positive eustachian tube insufflation.Methods 60 children with AH complicated with SOM from January 2017 to April 2018 were selected as the research subjects,they were randomly divided into control group and observation group,with 30 cases in each group.Children in two groups were given adenoidectomy with acupuncture,while children in observation group were given positive eustachian tube insufflation additionally.The clinical effect between two groups was compared.Results The overall effective rates at 2 weeks,3 and 6 months after operation(83.67%,87.76%and 93.88%)in observation group were slightly higher than those in control group(76.69%,82.98%and 87.23%),with no significant difference.The effective rates at 2 weeks,3 and 6 months after operation(75.76%,81.82%and 90.91%)in observation group were higher than those in control group(66.67%,73.33%and 80.00%),with significant difference between two groups(P<0.05).Conclusion The clinical effect of the puncture of the ear drum combined with positive eustachian tube insufflation in the treatment of children with AH complicated with SOM is exact,especially in the treatment of children with mucinous effusion.
