摘 要:目的比较运用无头空心加压螺钉与空心拉力螺钉治疗GardenⅠ、Ⅱ型股骨颈骨折的临床效果。方法将2017年9月至2018年9月郑州大学第一附属医院诊治的70例股骨颈骨折患者按随机数表法分为对照组和研究组。对照组采用空心拉力螺钉治疗,研究组采用无头空心加压螺钉治疗,每组35例。比较两组患者手术时间、术中出血量,术后9-12个月对两组患者进行定期门诊随访,比较患侧髋关节活动功能、股骨颈短缩情况。结果两组患者手术时间、术中出血量比较,差异无统计学意义(均P>0.05)。研究组股骨颈短缩移位发生率低于对照组,骨折愈合时间长于对照组,骨不连发生率高于对照组,差异有统计学意义(均P<0.05)。两组髋关节Harris评分比较,差异无统计学意义(P>0.05)。两组患者在固定失效、股骨头缺血坏死率比较,差异无统计学意义(均P>0.05)。结论与空心拉力螺钉固定方式治疗GardenⅠ、Ⅱ型股骨颈骨折相比,无头空心加压螺钉治疗方式并无明显优势。Objective To compare the clinical efficacy of headless cannulated compression screws and hollow lag screws in the treatment of Garden typeⅠand typeⅡfemoral neck fractures.Methods From September 2017 to September 2018,70 patients with femoral neck fracture diagnosed and treated in the First Affiliated Hospital of Zhengzhou University were randomly divided into control group and study group.The control group was treated with hollow tension screw,and the study group was treated with headless hollow compression screw,35 cases in each group.The operation time and intraoperative blood loss were compared between the two groups.The patients in the two groups were followed up regularly 9-12 months after operation.The hip joint movement function and the shortening of thigh neck were compared between the two groups.Results There was no significant difference in operation time and intraoperative blood loss between the two groups(all P>0.05).The incidence of shortening and displacement of thigh neck in the study group was lower than that in the control group,the fracture healing time was longer than that in the control group,and the incidence of nonunion was higher than that in the control group(all P<0.05).There was no significant difference in Harris score between the two groups(P>0.05).There was no significant difference between the two groups in fixation failure and avulsion rate of femoral head(all P>0.05).Conclusion Compared with hollow tension screw fixation in the treatment of Garden typeⅠand typeⅡfemoral neck fractures,headless hollow compression screw has no obvious advantage.
