摘  要:目的探讨一站式CT扫描在急性缺血性脑卒中(AIS)患者的诊断与预测溶栓后出血转化(HT)的价值。方法回顾性统计分析AIS患者缺血半暗带(IP)区与梗死核心(IC)区及健侧区脑灌注参数差异以及出血组(HT组)与非出血组(No-HT组)IP区灌注差异。结果AIS患者脑部IP区脑血流量(CBF)、脑血容量(CBV)、平均通过时间(MMT)及达峰时间(TTP)较IC区存在显著差异,CBF、MMT及TTP较健侧区存在显著差异;与No-HT组相比,HT组患者高血脂及高血压百分比显著偏高;HT组较No-HT组IP区CBF与CBV值显著降低。结论一站式CT扫描可快速评估AIS患者IP区和IC区,可初步预测HT,为提高AIS救治率、降低HT发生率提供客观及可靠的影像学依据。Objective To investigate the value o£one-stop CT scan in the diagnosis and prediction of hemorrhagic transformation(HT)in patients with acute ischemic stroke(AIS).Methods The difference of cerebral perfusion parameters between ischemic penumbra(IP)area and infarction core(IC)area and contralateral area were analyzed retrospectively in AIS patients.Then,the difference of IP perfusion was analyzed between bleeding group(HT group)and non-bleeding group(No-HT group).Results The cerebral blood flow(CBF).cerebral blood volume(CBV),mean transmit time(MMT)and time to peak(TTP)in IP of AIS patients were significantly different from those in IC area,while CBF,MMT and TTP were significantly different from those in healthy side area.Compared with No-HT group,the percentages o£hyperlipidemia and hypertension in HT group were significantly higher.The values of CBF and CBV in IP of HT group were significantly lower than those of No-HT group.Conclusion One-stop CT scan can quickly evaluate the IP area and IC area of AIS patients,and predict the HT irdtially,and provide an objective and reliable imaging basis for improving the AIS treatment rate and reducing the incidence of HT.
