摘　　要：目的探讨多层螺旋CT尿路造影(MSCTU)对膀胱癌术前T分期的诊断价值。方法选取MSCTU拟诊并经术后病理证实为膀胱癌的患者53例,判断MSCTU对于膀胱癌术前T分期的准确性及影响肿瘤分期的影像征象。结果MSCTU对膀胱癌术前T分期总准确率为87.50%,与术后病理分期具有高度的一致性。术后病理T分期与肿瘤大小、数量无关,差异无统计学意义(P>0.05);与肿瘤边界、基底及肾积水的发生有关,差异有统计学意义(P<0.05)。结论MSCTU对膀胱癌术前T分期诊断准确率高。膀胱癌影像表现边界不规则、宽基底及肾积水的发生可在一定程度上判断肿瘤浸润程度。Objective To evaluate the diagnostic value of multislice spiral CT urography(MSCTU)in preoperative T staging of bladder cancer.Methods 53 cases of bladder cancer confirmed by postoperative pathology were collected with MSCTU in our hospital,to evaluate the accuracy of MSCTU in preoperative T staging of bladder cancer and the imaging signs affecting tumor staging.Results The total accuracy of preoperative T staging of bladder cancer by MSCTU was 87.50%,which was highly consistent with postoperative pathological staging.There was no significant difference between pathological T staging and tumor size and quantity(P>0.05),but there was significant difference in tumor boundary,basal and hydronephrosis(P<0.05).Conclusion MSCTU has a high diagnostic accuracy for preoperative T staging of bladder cancer.The imaging signs of bladder cancer showed irregular boundary,wide basement and hydronephrosis,which could be used to judge the degree of tumor invasion to some extent.
