退变性腰椎侧凸与终板Modic改变之间的相关性研究

摘　　要：目的探讨退变性腰椎侧凸与终板Modic改变之间的相关性。方法收集行脊柱全长X线摄影和腰椎MR检查的患者影像学资料,共536例患者符合纳入标准,统计退变性腰椎侧凸、终板Modic改变的发生情况,将发生退变性腰椎侧凸的患者纳入研究组,其余患者纳入对照组。对退变性腰椎侧凸和终板Modic改变进行分级,分析两者之间的相关性。结果研究组2180个终板,对照组3180个终板。研究组中有534个终板发生Modic改变,对照组212个终板发生Modic改变,差异有统计学意义(χ^2=343.142,P<0.05);退变性腰椎侧凸分级与终板Modic改变分级之间呈正相关(r=0.69,P<0.05);顶椎、端椎内外侧面之间的终板Modic改变差异有统计学意义(χ^2=115.144,P<0.05;χ^2=89.759,P<0.05)。结论退变性腰椎侧凸与终板Modic改变具有正相关性,随着腰椎侧凸程度的加重,终板Modic改变的程度也随之加重。Objective To investigate the correlation between degenerative lumbar scoliosis and endplate Modic changes.Methods Imaging data of 536 patients who underwent both X-ray of total spine and lumbar MR examination were enrolled in our study.The incidence of degenerative lumbar scoliosis and endplate Modic changes were analyzed.Patients with degenerative lumbar scoliosis were included in the study group,and the remaining patients were included in the control group.The classifications of degenerative lumbar scoliosis and endplate Modic changes were determined,and the correlation between them was analyzed.Results Modic changes occurred in 534 endplates in the study group with a total of 2180 endplates,however only in 212 endplates in the control group with a total of 3180 endplates.The difference between two groups was statistically significant(χ^2=343.142,P<0.05).There was a positive correlation between the grades o£degenerative lumbar scoliosis and endplate Modic changes(r=0.69,P<0.05).The difference between interior and exterior vertebral Modic changes was statistically significant in both apical vertebrae and end vertebrae(χ^2=115.144,P<0.05;χ^2=89.759,P<0.05).Conclusion There is a positive correlation between degenerative lumbar scoliosis and endplate Modic changes.With the increasing grade of lumbar scoliosis,the endplate Modic changes also increased.
