兰州市城关区“三位一体”HIV/AIDS随访管理模式的探索与思考

摘　　要：目的提高甘肃省兰州市城关区艾滋病病毒感染者/艾滋病病人(简称HIV/AIDS)随访管理质量,探索适合甘肃和西部低流行地区"三位一体"(疾病预防控制中心-医院-社区)的HIV/AIDS随访管理模式。方法根据抗病毒治疗时间,将需随访的HIV/AIDS分成治疗时间不满3年、治疗时间3~5年、治疗5年以上和新发未治疗4组,分别由疾病预防控制中心、抗病毒治疗医院(兰州市肺科医院)、社区组织(诚爱家园)和社区卫生服务中心负责随访。结果经过5年(2014-2018年)的探索,兰州市城关区"三位一体"的HIV/AIDS随访管理模式基本形成,截止2018年底,城关区内近70.00%的HIV/AIDS随访管理工作已完成下沉,兰州市肺科医院、社区卫生服务中心、"诚爱家园"分别承担38.28%,3.83%和24.88%的随访工作,HIV/AIDS CD4检测比例达94.50%,配偶/固定性伴HIV抗体检测比例达96.60%,HIV/AIDS结核病检查比例达99.50%。结论"三位一体"紧密协作HIV/AIDS随访管理模式的建立,提升了HIV/AIDS随访质量,促进了抗病毒治疗比例提高、降低配偶/性伴HIV感染率和HIV/AIDS死亡率。objective To improve the quality of management model of follow-up management of HIV-infection/AIDS patients(brief in HIV/AIDS),establish a trinity HIV/AIDS follow-up by community,disease control and hospital.Methods According to the time of antiviral treatment,HIV/AIDS patients requiring follow-up were divided into four groups:treated less than three years,treated for 3-5 years,treated more than five years,and newly untreated HIV/AIDS.Follow-up visits were conducted by community health service center,community organizations(Love Home Sincerely),the center for disease control and prevention(CDC),and antiviral treatment hospital(Lanzhou Municipal Pulmonary Hospital),respectively.Results After five years(2014-2018),the trinity follow-up management pattern of HIV/AIDS formed elementarily as community-CDC-hospital.By the end of 2018,70.00%of the HIV/AIDS follow-up management was completed within the respective jurisdictions,and Lanzhou Municipal Pulmonary Hospital,community health service centers,Sincere Love Home undertook 38.28%,3.83%and 24.88%of the follow-up.The proportion reached 94.50%of HIV/AIDS CD4 determination,96.60%of spouse/fixed companion detection for HIV antibody,and 99.50%of HIV/AIDS tuberculosis examination.Conclusions Establishment of the trinity follow-up management model of HIV/AIDS makes the improvement of follow-up quality of HIV/AIDS,the proportion increase of antiviral treatment and decrease of the infection rate and mortality rate of HIV/AIDS in spouses/sexual partners.
