巨大疣状皮肤结核一例

摘　　要：47岁男性患者,全身多处皮肤反复出现增生性红斑20年,破溃伴疼痛1个月。皮损组织病理示:表皮角化不全,真皮浅层大量上皮样细胞增生,可见数个多核巨细胞及朗格汉斯巨细胞、周围大量淋巴细胞及浆细胞浸润。结核菌素纯蛋白衍生物(PPD)试验(±),结核杆菌特异性γ干扰素释放试验分析试验(TB-IGRA)试验(+),抗酸染色(-)。诊断:疣状皮肤结核。抗痨治疗3个月后,皮损消退。In a 47-year-old male patient,proliferative erythemas recurred in many parts of the skin for 20 years,with ulceration accompanied by pain for 1 month.The histopathologic features included hyperkeratosis and parakeratosis of squamous epithelium,epithelioid cell proliferation in superficial dermis with a few multinuclear macrophages and langerhans cells,as well as a large number of lymphocytes and plasma cells infiltrating peripherally.PPD test(±),TB-IGRA test(+),Acid-fast stain(-).Diagnosis:tuberculosis verrucosa cutis.After 3 months of anti-tuberculosis treatment,the lesions subsided.
