尼莫地平片联合多奈哌齐片治疗老年脑梗死的临床研究

摘　　要：目的观察尼莫地平片联合多奈哌齐片治疗老年脑梗死的临床疗效及安全性。方法将150例急性脑梗死患者随机分为对照组70例和试验组80例。对照组予以尼莫地平每次30 mg,tid,口服;试验组在对照组治疗的基础上,予以多奈哌齐每次5.0 mg,qn,睡前口服。2组患者均治疗24周。比较2组患者的美国国立卫生研究院卒中量表(NIHSS)评分、Barthel Index指数(BI指数)、神经元特异性烯醇化酶(NSE)、中枢神经特异性蛋白(S100β)和β-淀粉样蛋白(Aβ)水平,以及药物不良反应的发生情况。结果治疗后,试验组和对照组的NIHSS评分分别为(7.25±0.78)和(10.54±2.30)分,BI指数分别为(59.32±7.41)和(49.85±8.46),NSE分别为(6.65±0.99)和(10.07±2.12)ng·mL^-1,S100β分别为(369.45±44.56)和(454.36±36.94)ng·mL^-1,Aβ分别为(222.71±35.48)和(296.45±34.16)ng·mL^-1,差异均有统计学意义(均P<0.05)。2组患者治疗期间均未发生药物不良反应。结论尼莫地平片联合多奈哌齐片治疗老年脑梗死的临床疗效确切,其能显著改善患者血清NSE、S100β和Aβ水平,且安全性较高。Objective To observe the clinical efficacy and safety of nimodipine tablets combined with donepezil tablets in the treatment of senile cerebral infarction.Methods A total of 150 patients with acute cerebral infarction were randomly divided into control group(n=70 cases)and treatment group(n=80 cases).Control group was given nimodipine30 mg per time,tid,orally.Treatment group was given donepezil 5.0 mg per time,qn,orally before bedtime on the basis of control group.Both groups were treated for 24 weeks.The NIHSS score,Barthel index(BI),neuron specific enolase(NSE),central neuron specific protein(S100β),beta-amyloid protein(Aβ)and adverse drug reactions were compared between two groups.Results After treatment,the main indexes of treatment and control groups were compared:NIHSS were(7.25±0.78)and(10.54±2.30),BI were(59.32±7.41)and(49.85±8.46),NSE were(6.65±0.99)and(10.07±2.12)ng·mL^-1,S100βwere(369.45±44.56)and(454.36±36.94)ng·mL^-1,Aβwere(222.71±35.48)and(296.45±34.16)ng·mL^-1,the differences were statistically significant(all P<0.05).There were no adverse drug reactions occurred during the treatment in two groups.Conclusion Nimodipine tablets combined with donepezil tablets have a definitive clinical efficacy and safety in the treatment of senile cerebral infarction,which can significantly improve the levels of serum NSE,S100βand Aβ.
