摘  要:目的研究西罗莫司联合肝动脉化疗栓塞术(TACE)对兔肝移植瘤的影响。方法计算机断层扫描(CT)导引下穿刺大白兔植入VX2肿瘤细胞株制备肝移植瘤模型。将造模成功兔随机分2组:联合组和TACE组,每组8只。联合组:动脉灌注西罗莫司1 mg·kg^-1+表柔比星1 mg·kg^-1、丝裂霉素0.2mg·kg^-1、碘化油制成乳剂栓塞;TACE组:表柔比星1 mg·kg^-1、丝裂霉素0.2m·kg^-1、碘化油制成乳剂栓塞。用免疫印迹法检测血管内皮生长因子(VEGF)、诱导型一氧化氮合成酶(iNOS)和低氧诱导因子^-1α(HIF^-1α)蛋白相对表达水平,用免疫组化技术测量CD34表达水平并计算肿瘤组织微血管密度(MVD)。结果联合组和TACE组的VEGF蛋白相对表达水平分别为0.49±0.04,0.62±0.04;联合组和TACE组的iNOS蛋白相对表达水平分别为0.48±0.02,0.67±0.04;联合组和TACE组的HIF^-1α蛋白相对表达水平分别为0.66±0.03,0.70±0.03;联合组和TACE组的MVD分别为3.14±0.38,4.86±0.43。上述指标,组间比较差异均有统计学意义(均P<0.05)。结论动脉灌注西罗莫司联合TACE能够减少兔肝移植瘤的肿瘤新生血管的形成,提高介入栓塞治疗的效果。Objective To investigate the curative effect of rapamycin combined with transcatheter arterial chemoembolization(TACE)in the treatment of rabbit liver xenografted tumor.Methods New Zealand white rabbits were implanted with VX2 tumor lumps by computed tomography(CT)guided percutaneous liver puncture.The successful modeling of white rabbits were randomly divided into two groups:combination group and TACE group,8 rabbits in each group.Combination group:Arterial perfusion with rapamycin 1 mg·kg^-1+epirubicin 1 mg·kg^-1,mitomycin 0.2 mg·kg^-1 and iodized oil into tumor to embolize vessels;TACE group:infused the emulsion of epirubicin 1 mg·kg^-1,mitomycin0.2 mg·kg^-1 and iodized oil.The relative expressions of vascular endothelial growth factor(VEGF),inducible nitric oxide synthase(iNOS)and hypoxia inducible factor^-1α(HIF^-1α)were detected by Western blot.The expression of CD34 was detected by immunohistochemistry and the microvessel density(MVD)of tumor tissues was calculated.ResultsThe relative expression levels of VEGF in combined group and TACE group were 0.49±0.04,0.62±0.04;the relative expression levels of iNOS in combined group and TACE group were 0.48±0.02,0.67±0.04;the relative expression levels of HIF^-1αin combined group and TACE group were 0.66±0.03,0.70±0.03;the MVD in combined group and TACE group were 3.14±0.38,4.86±0.43.Comparing between combined group and TACE group,the differences of the factors were significant(all P<0.05).Conclusion Arterial infusion of rapamycin combined with TACE can reduce the formation of tumor angiogenesis and improve the curative effect of interventional embolization in the treatment of rabbit liver xenografted tumor.
