摘　　要：目的探讨临床药师在1例孕期服用卡马西平的产妇及产儿药学监护中的作用。方法临床药师通过分析1例孕期女性服用卡马西平对产儿的影响,探讨卡马西平的血药浓度与基因多态性的相关性及应用卡马西平控制癫痫发作的产妇是否可以进行母乳喂养,依托治疗药物监测、药物基因组学及循证药学的手段为患儿提供药学监护及辅助决策。结果与结论临床药师作为临床诊疗团队的一员,利用自身专业知识,可以为医师提供决策依据,有效改善患儿预后,减少药物不良反应的发生。Objective To explore the role of clinical pharmacists in the pharmaceutical care for a delivery women taking carbamazepine during pregnancy and her neonate.Methods By analyzing the effect of fetal carbamazepine exposure on a neonate,clinical pharmacists investigated the correlations between carbamazepine plasma concentrations and gene polymorphism,and explored whether women taking carbamazepine could perform breastfeeding.Relying on therapeutic drug monitoring,pharmacogenomics and evidence-based pharmacy,clinical pharmacists provided pharmaceutical care and decision supports.Results and conclusion As a member of the clinical team,clinical pharmacists use their professional knowledge to provide decision-making basis for physicians,which effectively improve the prognosis of patients and reduce the occurrence of adverse reactions.
