益气复脉通络方加减对不稳定型心绞痛心肌酶谱、sICAM-1和MMPs-9水平影响

摘　　要：目的:探讨益气复脉通络方加减治疗对不稳定型心绞痛患者心肌酶谱、可溶性细胞间黏附因子-1(sICAM-1)和基质金属蛋白酶-9(MMPs-9)水平影响,为不稳定型心绞痛患者的临床诊治提供参考。方法:选取不稳定型心绞痛患者100例,随机分为对照组与研究组,每组50例,两组患者均给西药常规治疗,研究组在以上治疗基础上给与益气复脉通络方加减治疗,治疗2周,观察患者治疗前后心绞痛发作次数、发作后持续时间变化,中医症候积分评分变化,乳酸脱氢酶(LDH)、磷酸肌酸激酶同工酶(CK-MB)、磷酸肌酸激酶(CK)变化,血清sICAM-1和MMPs-9变化。结果:(1)两组患者心绞痛发作次数、发作后持续时间治疗后明显少于(短于)治疗前(P<0.05),研究组治疗后明显少于(短于)对照组(P<0.05);(2)两组治疗后胸闷、心悸气短、胸疼、舌苔积分评分低于治疗前(P<0.05),研究组治疗后明显低于对照组(P<0.05);(3)两组治疗后LDH、CK-MB、CK、sICAM-1和MMPs-9水平低于治疗前(P<0.05),研究组治疗后明显低于对照组(P<0.05)。结论:益气复脉通络方加减治疗对不稳定型心绞痛患者有助于快速改善症状,促进心肌酶、sICAM-1和MMPs-9水平下降。Objective:To investigate the effect of modified Yiqi Fumai Tongluo Decoction for myocardial enzymes and its influence on sICAM-1 and MMPs-9 in patients with unstable angina pectoris and provide the basis for clinic treatment of patients with unstable angina pectoris. Methods: We selected 100 cases of patients with unstable angina pectoris. They were randomly divided into control group(50 cases) and observation group(50 cases) and the two groups received conventional western medicine treatment. The observation group additionally used Modified Yiqi Fumai Tongluo Decoction. Both were treated for 2 weeks. We observed the times of angina attacks,the duration changes after the onset,changes in TCM syndromes score,Lactic dehydrogenase(LDH),phosphocreatine kinase isoenzyme(CK-MB),the change of phosphocreatine kinase(CK),serum sICAM-1 and MMPs-9 before and after treatment. Results:(1)Times of angina attacks and the duration changes after the onset were significantly lower/shorter than those before treatment(P<0.05). Those of the observation group after treament were significantly shorter than those of the control group(P<0.05).(2)Scores of chest tightness, palpitation, shortness of breath, chest pain and tongue coating were significantly lower than those before treatment(P<0.05). Those of the observation group after treatment were significantly lower than those of the control group(P<0.05).(3)LDH,CK-MB and CK,sICAM-1,MMPs-9 after treatment were significantly lower than those before treatment(P<0.05). Those of the observation group were significantly lower than those of the control group after treatment(P<0.05). Conclusion:Modified Yiqi Fumai Tongluo Decoction can rapidly improve symptoms and promote the declines in myocardial enzymes, sICAM-1 and MMPs-9 levels for patients with unstable angina pectoris.
