益肾调经法联合人工周期治疗卵巢功能减退临床疗效及对子宫内膜容受性的影响

摘　　要：目的:观察益肾调经法联合人工周期治疗卵巢功能减退的临床疗效及对子宫内膜容受性的影响。方法:将120例肾虚血瘀型卵巢功能衰竭患者分为研究组和对照组,每组60例。对照组采用人工周期替代疗法治疗,研究组在对照组治疗的基础上,配合中药益肾调经汤治疗。两组均治疗3个疗程。比较两组治疗疗效,治疗前后中医症候积分,治疗前后性激素水平,治疗前后治疗前后OVD、AFC、RI水平和子宫内膜容受性改善情况。结果:研究组患者治疗总有效率为93.33%高于对照组治疗总有效率的80.00%(P<0.05);研究组治疗后烘热汗出、烦躁易怒、头晕耳鸣、失眠健忘、腰膝酸软和中医症候总积分低于对照组(P<0.05);研究组治疗后性激素FSH和LH水平低于对照组,E2、AMH水平高于对照组(P<0.05);研究组治疗后AFC水平高于对照组,RI水平低于对照组(P<0.05);研究组治疗后LIF、αVβ3和gly-codelin水平高于对照组(P<0.05)。结论:益肾调经汤治疗卵巢储备功能减退疗效确切,能有效的降低患者的中医症候积分,改善性激素,增加卵巢血流量,提高卵巢储备功能和子宫内膜容受性,和人工周期联合应用,可提高治疗疗效,值得临床推广应用。Objective: To observe the clinical efficacy of kidney-tonifying regulating menstruation method combined with artificial cycle in the treatment of ovarian hypofunction and the effect on endometrial receptivity. Methods: Totally 120 cases of ovarian failure with kidney deficiency and blood stasis were divided into study group and control group, 60 cases in each group. The control group was treated with artificial periodic replacement therapy, and the study group was treated with traditional Chinese medicine Yishen Tiaojing Decoction on the basis of the treatment of the control group. Both groups were treated for 3 courses. The therapeutic effects of the two groups were compared, including TCM syndrome scores, sex hormone levels, OVD, AFC, RI levels before and after treatment, and endometrial receptivity improvement. Results: The total effective rate in the study group was 93.33%, which was higher than 80.00% in the control group(P<0.05). After treatment, the total scores of hot sweating, irritability, dizziness, tinnitus, insomnia, forgetfulness, waist and knee tenderness and TCM symptoms in the study group were lower than those in the control group(P<0.05). After treatment, the levels of FSH and LH in the study group were lower than those in the control group, and the levels of E2 and AMH were higher than those in the control group(P<0.05). AFC level in the study group was higher than that in the control group, and RI level was lower than that in the control group(P<0.05). LIF, V3 and gly-codelin levels were higher in the study group than those in the control group(P<0.05). Conclusion: Yishen Tiaojing Decoction has a definite curative effect in the treatment of hypofunction of ovarian reserve, which can effectively reduce the TCM syndrome score of patients, improve sexual hormones, increase ovarian blood flow, improve ovarian reserve function and endometrial recepbility, and combined with artificial cycle, can improve the curative effect and is worthy of clinical promotion and application.
