清热通腑活血方联合灌肠对重症急性胰腺炎患者血清生化指标影响

摘　　要：目的 探讨清热通腑活血方联合灌肠对重症急性胰腺炎患者血清生化指标的影响.方法 选取2014年6月至2018年6月重症急性胰腺炎患者46例,随机分为观察组与对照组各23例.对照组行常规治疗,观察组在此基础上使用清热通腑活血方联合灌肠方法治疗,比较两组患者临床效果及血清生化指标并于1个月后进行随访.结果 观察组总体有效率95.7%明显高于对照组65.2%;经治疗,两组患者淀粉酶、降钙素原及肌酐水平均有所下降;两组患者IL-6、TNF-α及IL-8水平均有所下降;两组患者CD3+与CD4+水平均有所升高而CD8+水平下降,且观察组变化更显著;两组患者复发情况差异无统计学意义.结论 在常规治疗基础上使用清热通腑活血方灌肠能够减轻患者炎症反应,提高免疫功能,且不会对患者预后造成影响.Objective To study the effect of Qingre Tongfu Huoxue Decoction and enema on serum biochemical indexes in severe acute pancreatitis.Methods 46 patients with severe acute pancreatitis admitted to our hospital from June 2014 to June 2018 were randomly divided into observation group and control group with 23 cases in each group.The control group was given routine treatment,while the observation group was given Qingre Tongfu Huoxue Decoction combined with enema.The clinical effects and serum biochemical indexes of the two groups were recorded and followed up after 1 month.Results The total effective rate of the observation group was 95.7%,which was significantly higher than that of the control group(65.2%).After treatment,the levels of amylase,procalcitonin and creatinine in the two groups were decreased;the levels of IL-6,TNF-alpha and IL-8 in the two groups were decreased;the levels of CD3+and CD4+in the two groups were increased,while the level of CD8+was decreased,and the observation was made.Group changes were more significant;there was no significant difference in recurrence between the two groups.Conclusion On the basis of routine treatment,Qingre Tongfu Huoxue Decoction plus enema can reduce inflammation and improve immune function,and will not affect the prognosis of patients.
