同剂量不同浓度罗哌卡因TAP对剖宫产孕妇术后炎性因子的影响

摘　　要：目的 探讨同剂量浓度差异下罗哌卡因行腹横筋膜阻滞(TAP)对剖宫产孕妇术后炎性因子表达的影响.方法 将150例剖宫产孕妇随机分为高浓度罗哌卡因TAP组(H组)、中等浓度罗哌卡因TAP组(M组)和低浓度罗哌卡因TAP组(L组).各组予对应浓度罗哌卡因行TAP,记录各组患者术后1h、4h、12h、24hVAS评分,同时于术后第1、3、7天检测各组患者C-反应蛋白、白细胞计数、血清IL-6水平及三组患者术后转归.结果 三组患者术后各时间点VAS评分差异无统计学意义(P>0.05);三组患者术后组间比较,L组炎性因子水平低于H、M组(P<0.05),术后第1、3、7天各炎性指标组内比较,差异有统计学意义(P<0.05);首次下地时间L组短于H、M组(P<0.05).结论 相比高浓度、中等浓度罗哌卡因TAP,低浓度罗哌卡因(0.25%)TAP对剖宫产孕妇术后炎性因子表达影响更小.Objective To study the effects of inflammatory factors level after caesarean section by using different concentrations in transversus abdominis plane block.Methods 150 patients underwent caesarean section randomly divided into three groups:high concentration doze group(H group),middle concentration doze group(M group)and low concentration doze group(L group).Each group received transversus abdominis plane blcok(TAP)after operation by different concentrations of ropivacaine.VAS scores were recorded at 1h,4h,12h and 24h.C-Reactive protein,white blood cell count and interleukin-6 were tested at 1d,3d and 7d after operation.Results The VAS scores had no differences within three groups at 1d,3d and 7d after caesarean section(P>0.05).Compared with three groups,the inflammatory factor level in group L was lower than group H and M(P<0.05).There were statistically differences in inflammatory factor level among three groups at 1,3 and 7 days after surgery(P<0.05).The patients first down time after operation was shorter in group L than other two groups(P<0.05).Conclusion Compared with the groups using high and middle concentrations in transversus abdominis plane block,the patients who received TAP with low concentration(0.25%)has low level of inflammatory factors.
