摘 要:目的探讨先天性心脏病术后早期婴幼儿患者动态肺顺应性的变化情况及其影响因素。方法选择2012-2016年期间,择期体外循环下行先天性心脏病手术的<5岁患儿190例,以正性肌力药物评分(IS)分为轻重两组。采用PB840呼吸机监测术毕返ICU即刻、2、4和6h时的肺顺应性、气道峰压和血气指标的变化。结果顺应性与年龄(r=0.768,P=0.000)、身高(r=0.766,P=0.000)、体重(r=0.764,P=0.000)和体表面积(r=0.53,P=0.000)等高度相关,在轻重患者间有显著差异结论以呼吸机动态监测肺顺应性,结合血气换气功能指标,有利于判断病情,并可以为临床脱机提供依据。Objective To investigate the changes and influencing factors of dynamic lung compliance in early postoperative infants with congenital heart disease.Methods From 2012 to 2016,190 cases of children<5 years old who underwent surgery for congenital heart disease under elective extracorporeal circulation were selected and were divided into two groups according to the positive inotropic drug score(IS).PB840 ventilator was used to monitor the changes of lung compliance,airway peak pressure and blood gas index at immediate,2,4 and 6 h after ICU.Results Compliance was highly correlated with the age(r=0.768,P=0.000),height(r=0.768,P=0.000),weight(r=0.764,P=0.000) and body surface area(r=0.53,P=0.000),with significant differences between the severe and severe patients.Conclusion Monitoring dynamic lung compliance by ventilator combined with blood gas exchange function index is helpful to judge the patient condition and can provide the basis for clinical off-line treatment.
