止痛贴辅助治疗结核性胸膜炎临床研究及对炎症因子的影响

摘　　要：目的:观察止痛贴辅助治疗结核性胸膜炎的疗效对胸水炎症因子的影响。方法:将结核性胸膜炎患者62例采用随机法分为2组31例。对照组为西医规范疗法,治疗组在对照组治疗方案基础上给予止痛贴辅助治疗,治疗期为21天。观察2组治疗前后的临床症状与体征、疗效、胸腔积液中炎症因子[乳酸脱氢酶(LDH)、肿瘤坏死因子-α(TNF-α)]水平。结果:治疗后,2组中医临床症状体征评分均较治疗前下降(P<0.05)。治疗组在发热、咳嗽、胸闷、胸胁痛等方面症状体征减轻程度优于对照组(P<0.05)。治疗后,治疗组总有效率为96.77%,对照组为83.87%,治疗组疗效优于对照组(P<0.05)。治疗后,2组胸水LDH、TNF-α水平下降(P<0.05)。且治疗组胸水LDH、TNF-α较对照组下降更为显著(P<0.05)。结论:止痛贴联合抗痨方案治疗结核性胸膜炎疗效显著。Objective:To observe the curative effect of analgesic plaster for tuberculous pleurisy and its effect on inflammatory factors in pleural effusion.Methods:A total of 62 cases of patients with tuberculous pleurisy were randomly divided into two groups,31 cases in each group.The control group was treated with routine western medicine therapy,and the treatment group was additionally treated with analgesic plaster based on the treatment of the control group.Both groups were treated for 21 days.Before and after treatment,the clinical symptoms and signs,the curative effect and the levels of inflammatory factors in pleural effusion including lactate dehydrogenase(LDH) and tumor necrosis factor-α(TNF-α) were observed Results:After treatment,the scores of clinical symptoms and signs in Chinese medicine in the two groups were decreased when compared with those before treatment(P <0.05).The decrease of symptoms and signs including fever,cough,chest distress as well as chest and hypochondrium pain in the treatment group was larger than that in the control group(P <0.05).After treatment,the total effective rate was 96.77% in the treatment group,higher than that of 83.87% in the control group(P <0.05).After treatment,the levels of LDH and TNF-a in pleural effusion in the two groups were decreased(P<0.05),and the decrease of the above levels in the treatment group was more significant than that in the control group(P <0.05).Conclusion:The therapy of analgesic plaster combined with anti-tuberculosis has significant curative effect in treating tuberculous pleurisy.
