和胃通降汤联合兰索拉唑治疗非糜烂性反流病肝胃郁热证临床研究  

Clinical Study on Hewei Tongjiang Tang Combined with Lansoprazole for Non-erosive Reflux Disease with Liver-stomach Dampness-heat Syndrome

摘　　要：目的:观察和胃通降汤联合兰索拉唑治疗非糜烂性反流病(NERD)肝胃郁热证的临床疗效。方法:将98例NERD肝胃郁热证患者按随机数字表法分为观察组和对照组各49例。对照组给予兰索拉唑肠溶胶囊治疗,观察组在对照组基础上加用和胃通降汤,2组均连续治疗8周。比较2组临床疗效,治疗前后记录患者的胃食管反流病问卷(GerdQ)评分和烧心、反食、非心源性胸痛、反酸评分。结果:观察组总有效率91.84%,高于对照组的69.39%,差异有统计学意义(P<0.05)。治疗后,2组阳性症状、阴性症状、阳性影响计分和GerdQ总分均较治疗前降低(P<0.01);观察组阳性症状、阴性症状、阳性影响计分和GerdQ总分均低于对照组(P<0.01)。治疗后,2组烧心、反食、非心源性胸痛、反酸积分及总积分均较治疗前下降(P<0.01);观察组上述各项症状积分均低于对照组(P<0.01)。结论:和胃通降汤联合兰索拉唑治疗NERD肝胃郁热证可减轻患者的临床症状,改善生活质量,临床疗效优于单纯西药治疗。Objective:To observe the clinical effect of Hewei Tongjiang tang combined with lansoprazole for non-erosive reflux disease(NERD)with liver-stomach dampness-heat syndrome.Methods:A total of 98 cases of NERD patients with liver-stomach dampness-heat syndrome were divided into the observation group and the control group according to the random number table method,49 cases in each group.The control group was treated with lansoprazole enteric-coated capsules,and the observation group was additionally treated with Hewei Tongjiang tang based on the treatment of the control group.Both groups were treated for eight weeks.The clinical effect and the scores of Gastroesophageal Reflux Disease Questionnaire(GerdQ),heartburn,food regurgitation,non-cardiac chest pain and acid regurgitation before and after treatment in the two groups were compared.Results:The total effective rate was91.84%in observation group,higher than that of 69.39%in the control group,the difference being significant(P<0.05).After treatment,the scores of positive syndromes,negative syndromes and positive effect as well as the total scores of GerdQ in the two groups were decreased when compared with those before treatment(P<0.01);the above scores in the observation group were lower than those in the control group(P<0.01).After treatment,the scores of heartburn,food regurgitation,non-cardiac chest pain and acid regurgitation as well as the total scores in the two groups were decreased when compared with those before treatment(P<0.01);the above symptoms scores in the observation group were lower than those in the control group(P<0.01).Conclusion:The therapy of Hewei Tongjiang tang combined with lansoprazole in treating NERD with liver-stomach dampness-heat syndrome can alleviate the clinical symptoms of patients and improve their quality of life,whose clinical effect is superior to that of simple western medicine treatment.
