清热利湿复方联合激素治疗风湿痹阻型系统性红斑狼疮临床研究

摘　　要：目的:观察清热利湿复方联合激素治疗风湿痹阻型系统性红斑狼疮(SLE)的临床疗效。方法:将风湿痹阻型SLE患者196例随机分为对照组和观察组,每组98例;对照组给予口服泼尼松片、硫酸羟氯喹片联合甲氨蝶呤片治疗,观察组给予口服醋酸泼尼松片联合清热利湿复方治疗,2组均连续治疗8周;观察2组患者临床疗效、中医证候及狼疮疾病活动指数(SLE-DAI)积分变化,比较2组患者治疗前后24h尿蛋白、抗ds-DNA抗体、补体C3、补体C4、红细胞沉降率(ESR)的差异,记录2组不良反应发生率。结果:总有效率观察组为92.9%,对照组为62.2%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组中医证候积分、SLE-DAI积分、24h尿蛋白、抗ds-DNA抗体水平、ESR均较治疗前显著降低,补体C3、C4水平显著升高,差异均有统计学意义(P<0.05);且观察组上述各指标改善较对照组更显著(P<0.05)。不良反应发生率观察组为9.18%,对照组为38.78%,2组比较,差异有统计学意义(P<0.05)。结论:清热利湿复方联合激素治疗风湿痹阻型红斑狼疮疗效显著,能有效改善患者临床症状及SLE-DAI积分,降低相关实验室指标,减轻激素不良反应,值得临床推广应用。Objective:To observe the clinical effect of the therapy of Qingre Lishi Compound combined with hormone for systemic lupus erythematosus(SLE) of wind-dampness obstruction type.Methods:A total of 196 patients with SLE of wind-dampness obstruction type were randomly divided into the control group and the observation group,98 patients in each group.The control group was treated with oral administration of prednisone tablets,hydroxychloroquine sulfate tablets and methotrexate tablets,and the observation group was treated with oral administration of prednisone acetate tablets combined with Qingre Lishi Compound.Both groups were treated for eight weeks.The changes in clinical effect and the scores of Chinese medicine syndrome and Systemic Lupus Erythematosus-Disease Activity Index(SLE-DAI) of patients in the two groups were observed.The differences in 24 h urine protein,anti-ds-DNA antibody,complement C3,complement C4,and erythrocyte sedimentation rate(ESR) between the two groups before and after treatment were compared,and the incidence of adverse reactions in the two groups were recorded.Results:The total effective rate was 92.9% in the observation group and62.2% in the control group,the difference being significant(P <0.05).After treatment,Chinese medicine syndrome score,SLE-DAI score,24 h urine protein,the level of anti-ds-DNA antibody and ESR in the two groups were significantly decreased when compared with those before treatment,while levels of complement C3 and C4 were significantly increased,differences being significant(P <0.05).The improvement of the above indexes in the observation group was more significant than that in the control group(P <0.05).The incidence of adverse reactions was 9.18% in the observation group and38.78% in the control group,the difference being significant(P <0.05).Conclusion:The therapy of Qingre Lishi Compound combined with hormone has significant clinical effect in treating patients with SLE of wind-dampness obstruction type,which can effectively improve their clinical symptoms and t
