益肾通痹汤治疗腰椎管狭窄症临床研究

摘　　要：目的:观察益肾通痹汤治疗腰椎管狭窄症的临床效果。方法:将106例腰椎管狭窄症患者随机分为治疗组和对照组各53例,治疗组口服益肾通痹汤治疗,对照组采用甘露醇注射液静脉给药治疗,2组均观察用药3周。治疗前后评定腰部、腿部的疼痛视觉模拟评分法(VAS)评分,评定日本矫形外科协会(JOA)评分并根据JOA评分改善率评价疗效。结果:治疗3周后,治疗组总有效率94.34%,高于对照组的79.24%,差异有统计学意义(P<0.05)。2组腰部、腿部的VAS评分均较治疗前下降(P<0.05),治疗组腰部、腿部的VAS评分均低于对照组(P<0.05)。2组JOA评分均较治疗前上升(P<0.05),治疗组JOA评分高于对照组(P<0.05)。治疗组的JOA评分改善指数和改善率均高于对照组(P<0.05)。结论:相比甘露醇注射液,益肾通痹汤可更为有效地缓解腰椎管狭窄症患者的相关临床症状,减轻疼痛程度,改善躯体功能状况,提高疗效。Objective:To observe the clinical effect of Yishen Tongbi tang for lumbar spinal stenosis.Methods:Divided 106 cases of patients with lumbar spinal stenosis into the treatment group and the control group randomly,53 cases in each group.The treatment group received the oral administration of Yishen Tongbi tang,while the control group received the intravenous administration of mannitol injection.The observation of administration in both groups lasted for 3 weeks.Before and after treatment,visual analogue scale(VAS)scores in the waist and legs and Japanese orthopaedic association(JO A)scores,and evaluated the clinical effect according to the improvement rate of JOA scores.Results:After 3 weeks of treatment,the total effective rate was 94.34%in the treatment group,being higher than 79.24%in the control group,difference being significant(P<0.05).The VAS scores in the waist and legs of both groups were lower than those before treatment(P<0.05),the VAS scores in the waist and legs of the treatment group were lower than those of the control group(P<0.05).The JOA scores in both groups were higher than those before treatment(P<0.05),the JOA scores in the treatment group were higher than those in the control group(P<0.05).The improvement index and the improvement rate of JOA scores in the treatment group were respectively higher than those in the control group(P<0.05).Conclusion:In comparison with mannitol injection,Yishen Tongbi tang can relieve the relevant clinical symptoms of patients with lumbar spinal stenosis,alleviate the degree of pain and improve physical function more effectively,thus to promote the therapeutic effect.
