疏肝理气散结汤加减对乳腺增生病患者性激素的影响及临床研究

摘　　要：目的:观察疏肝理气散结汤加减对乳腺增生病(HMG)患者性激素的影响及临床疗效研究。方法:将HMG患者104例,随机分为对照组与治疗组各52例,对照组给予雌激素受体拮抗剂他莫昔芬治疗,治疗组在此基础上加用疏肝理气散结汤中西医结合治疗。观察治疗前后2组患者疼痛视觉模拟评分法(VAS)评分、性激素指标改变[雌二醇(E2)、孕酮(P)、催乳素(PRL)],比较2组临床疗效。结果:治疗后,2组VAS评分较前改善(P<0.05),但与对照组比较,治疗组改善更显著,差异有统计学意义(P<0.05)。治疗后,2组性激素指标均较前改善(P<0.05),治疗后组间比较,差异有统计学意义(P<0.05)。治疗组总疗效为86.5%,对照组为71.2%,2组比较,差异有统计学意义(P<0.05)。结论:在服用雌激素受体拮抗剂他莫昔芬基础上给予疏肝理气散结汤加减联合治疗HMG,能够很好的改善患者临床症状、乳腺功能以及性激素水平。Objective:To observe the effect of modified Shugan Liqi Sanjie tang on sex hormone in patients with hyperplasia of mammary glands(HMG)and the study on its clinical effect.Methods:Divided 104 cases of HMG patients into the control group and the treatment group randomly,with 52 cases in each group.The control group was treated with the estrogen receptor antagonist tamoxifen and the treatment group was additionally given Shugan Liqi Sanjie tang combined with integrated Chinese and western medicine therapy.Observed the scores of the pain visual analogue scale(VAS)and the changes of the indexes of sex hormone[estradiol(E2),progesterone(P)and prolactin(PRL)]in the two groups before and after treatment and compared the clinical effect in the two groups.Results:After treatment,VAS scores in the two groups were improved when compared with those before treatment(P<0.05),while the improvement in the treatment group was more evident when compared with that in the control group,difference being significant(P<0.05).After treatment,the indexes of sex hormone in the two groups were improved when compared with those before treatment(P<0.05),and there was significant difference being found in the comparison between the two groups after treatment(P<0.05).The total curative effect was 86.5%in the treatment group and 71.2%in the control group,compared the two groups,difference being significant(P<0.05).Conclusion:The additional application of the combination of modified Shugan Liqi Sanjie tang on the basis of the administration of the estrogen receptor antagonist tamoxifen for HMG can well improve the clinical symptoms,thymus function and sex hormone levels of the patients.
