血清半乳凝集素-3对慢性心力衰竭的诊断价值

摘　　要：目的:探讨血清半乳凝集素-3在慢性心力衰竭(CHF)中的诊断价值。方法:选取CHF患者71例和健康志愿者29例,按照NYHA分级将CHF患者分为:心功能Ⅱ级组14例,心功能Ⅲ级组32例,和心功能Ⅳ级组25例。检测血清半乳凝集素-3浓度和脑钠肽(BNP)水平。结果:心功能Ⅱ~Ⅳ级患者的半乳凝集素-3、BNP水平均显著高于对照组,且各亚组之间的半乳凝集素-3、BNP水平亦有显著差异(P<0.01)。相关分析显示:半乳凝集素-3与BNP、左室前后径(LVD)、NYHA心功能分级呈正相关(r=0.967、0.357、0.850,P<0.001、=0.002、<0.001);与左室射血分数(LVEF)呈负相关(r=-0.346,P=0.003)。结论:CHF患者血清中半乳凝集素-3的水平与心功能衰竭的严重程度相关,提示半乳凝集素-3有利于心力衰竭的诊断以及评估其严重程度。Objective:This study aimed to discuss the diagnostic value of plasma Galectin-3 in chronic heart failure(CHF).Method:Seventy-one patients with CHF and twenty-nine healthy subjects were recruited.CHF patients were divided into Class Ⅱ group(n=14),Class Ⅲ group(n=32),Class Ⅳ group(n=25) with New York Heart Association(NYHA) functional classification.Serum Galectin-3 and BNP were measured.Result:Significant increases of Galectin-3 and BNP were detected in HF patients with NYHA Class Ⅱ-Ⅳ compared with those of the control group.The levels of Galectin-3 and BNP among those various subgroups have significant differences respectively(all P<0.01).The results from the correlation analysis showed that Galectin-3 was positively related to BNP,LVD and NYHA class(r=0.967,0.357,0.850,P<0.001,=0.002,<0.001 respectively),but inversely related to LVEF(r=-0.346,P=0.003).Conclusion:Level of serum Galectin-3 is correlated with severity of heart failure.Galectin-3 is helpful to the diagnosis and assessing severity of heart failure.
