脊髓空洞症合并重度混合型睡眠呼吸暂停和右心衰竭1例

摘　　要：1病例资料患者,女,55岁,主因呼吸困难1个月,加重伴双下肢水肿3周入院。入院前1个月出现呼吸困难、活动耐量下降,检查发现血压升高.予降压治疗;入院前3周呼吸困难加重,合并咳嗽、咳白痰,伴眼睑、颜面、双下肢可凹性水肿,予抗感染治疗无好转;近1个月体重增加10 kg。既往发现脊髓空洞症35年、Chiari畸形I型(小脑扁桃体下疝)13年、脊柱胸段左侧弯。Simply right heart failure with pulmonary hypertension probably caused by sleep disorders was rare and hard to notice.Realized central nervous system malformation might relate to sleep orders still need help of multiple disciplinary team.The case reports a 55-year-old female diagnosed as right heart failure with pulmonary hypertension.As further symptoms were gathered,alveolar hypopnea syndrome and mixed sleep apnea hypopnea syndrome were diagnosed.Through the discussion of multiple disciplinary team,her past medical history of syringomyelia was the probably cause of those sleep disorders.
