中药外治法治疗单纯胸腰椎压缩性骨折

摘　　要：目的:在中医骨折“三期瓣证”理论指导下,观察运用中药外治法治疗单纯胸腰椎压缩性骨折的临床疗效。方法:收集2017年1月至2018年12月在静安区石门二路街道社区卫生服务中心就诊的单纯胸腰椎压缩性骨折患者84例,随机分为对照组41例和治疗组43例。对照组给予卧床静养为主,配合腰部垫枕、腰背肌功能锻炼及早期口服消炎镇痛药治疗;治疗组采用腰部垫枕卧床、腰背肌功能锻炼、自制中医膏药于骨折早中晚三期瓣证外敷治疗。2周为1个疗程,持续4个疗程,观察两组的疼痛视觉模拟(VAS)评分和Oswestry功能障碍指数(ODI)评分,比较两组总体疗效。结果:两组治疗后VAS、ODI评分均低于同组治疗前(P<0.05),且治疗组的VAS、ODI评分低于对照组,差异有统计学意义(P<0.05)。治疗组总有效率为95.3%,明显高于对照组的80.5%(P<0.05)。结论:运用中药外治法于骨折早中晚三期瓣证治疗单纯胸腰椎压缩性骨折临床疗效显著,适合在基层医院推广应用。Objective:To observe the clinical efficacy of external therapy of traditional Chinese medicine(TCM)in the treatment of simple thoracolumbar vertebral compression fractures under the guidance of the theory of“three stage dialectics”of fracture in traditional Chinese medicine.Methods:From January 2017 to December 2018,84 patients with simple thoracolumbar compression fractures treated in Shimenerlu Community Health Service Center of Jingan District were collected and randomly divided into a control group with 41 cases and a treatment group with 43 cases.The control group was mainly given bed rest,combined with the waist pillow,back muscle functional exercise and early oral anti-inflammatory and analgesic treatment.The treatment group was treated with lumbar bolster bed,back muscle function exercise,and self-made Chinese plaster external application in the fracture early,middle and late stages according to TCM syndrome differentiation.Two weeks were for 1 course of treatment,lasting 4 courses.The pain visual analogue(VAS)score and the Oswestry dysfunction index(ODI)score were observed in the two groups,and the overall efficacy of the two groups was compared.Results:After treatment,the VAS and ODI scores were lower than those in the same group before treatment(P<0.05),and the VAS and ODI scores of the treatment group were lower than those of the control group(P<0.05).The total effective rate of the treatment group was 95.3%,which was significantly higher than that of the control group 80.5%(P<0.05).Conclusion:The external therapy of TCM is effective in the treatment of simple thoracolumbar compression fractures in the early,middle and late stages of fractures by dialectical treatment,which is suitable for promotion and application in primary hospitals.
